When the Palmer leaves the room after talking with Sherry, the frame on the wall is opened as well as the safe. When he leaves though, outside, from the mirror reflection you see the frame is closed.
When Sherry Palmer is on the phone just before she opens the safe, the time on her watch is around 12. when the time line is about 5.30.
When Jack tried to call the safe-house, he said that no one was picking up. Previously, when Tony tried to call the house, he said the phone wasn't even ringing, presumably because the phone-line was cut.
Continuity error when Jack and Nina are on the escalator at the California Plaza: in the medium shots, a man and a woman are seen a few steps in front of them. In the long shot, Jack and Nina are the only ones on the escalator.

Factual errors

Palmer puts the tape in an envelope and addresses it to "District Attorney, State of California". However a district attorney is the chief prosecutor at the county level, not the entire state. If Palmer wants the tape to go to the state of California's chief prosecutor then it should be addressed to the Attorney General of California.

Crew or equipment visible

When George Mason is asking Tony Almeida if they got anything back from forensics, you can see a camera over his left shoulder.

Character error

Experienced federal agents do not touch their ears whenever they are talking into their microphones, it is a dead giveaway that they are wired. It is definitely not a mistake Jack would make since he used to be a Delta Force operator, they used hidden mics constantly and are trained experts at blending into crowds and not being noticed.
