When Linda is about to get the injection, Dr. Roth is first on her right side, then when the camera goes to a further view, he's seen on her left side, then he actually gives her the injection he's on her right side again.

Continuity

After Jim takes the kids to school and tells Claire he won't meet her, he calls Linda at home to say he wants to work things out. Linda is calling him and Jim looks at his phone and sees she is calling. The time on the phone is 3:45 pm. Yet this is all supposed to be first thing in the morning.
When Linda is writing out the calendar, and adds the red "CUTS" element to Tuesday, you can see a red entry on the adjacent-to-the-right day (Wednesday). But when the camera again looks in from the top, Wednesday is clear.
When Linda is about to get the injection, Dr. Roth is first on her right side, then when the camera goes to a further view, he's seen on her left side, then he actually gives her the injection he's on her right side again.
When Linda is walking toward the trash can, the level of tissues on top of the pile changes when she actually reaches in for what she is looking for.
At the funeral, the car of the woman behind the tree moves up and back between scenes.

Factual errors

Modern repair of facial injuries would not involve large black thread as used on the daughter, especially on a young child. Deep suturing, skin adhesives, dissolvable clear thread, and other techniques would both minimize future scarring and reduce the visual impact whilst healing.
The priest is reading aloud about a woman who had a premonition about her town being wiped out. He says they were wiped out by a "gale-force hurricane". There is no such thing. Gale-force winds are being 39-54 mph. Hurricanes have winds in excess of 74 mph.
When informing relatives about a loss, a police officer never tells this at the doorstep. He will always ask to come inside and instruct the relatives to sit down before the information is delivered; also, there are usually two officers instead of only one.
Linda looks in the yellow pages for Dr Roth's phone number. But prescription medicine containers have the prescribing doctor's phone number on them. Although at this point Linda had not yet received the prescription in real time, she just remembered the bottle from the medicine cabinet during a premonition.
Towards the end of the film, Jim stalls his car just beneath the brow of the hill. If he had removed the handbrake then the car would have rolled out of the way of the lorry which ultimately kills him.

Incorrectly regarded as goofs

On day #1 of the movie, Jim dies. Older daughter Bridgette should have stitches on her face from going through the glass sliding door while her father was alive earlier that week, but her face is completely clear--no scarring, no stitches.

However the way the "time travel" works in the movie is that Linda re-lives the days leading up to Jim's accident, changing what happens on those days. Only the second time around did Bridgette cut her face, so from the perspective of the other characters, her face on Friday (funeral day) is suddenly cut up.
On day #1 the police came to her home to inform her of her husband's death. Then viewers find out she was at the scene of the crash and witnessed her husband's death. So a police officer would not have had to come to tell her of the accident.

However the way the "time travel" works in the movie is that Linda re-lives the days leading up to Jim's accident, changing what happens on those days. Only the second time around was Linda at the scene; in other words it happened the second time she experienced Wednesday, whereas the officers told her of Jim's death the first time she experienced Thursday.
Linda pours her prescribed medicine pills down the sink drain instead of throwing it in the garbage. Medicine that is poured down the drain can be dangerous because it could contaminate lakes and streams, which may end up in drinking water. However, while this is a mistake on the part of the character, it is something many people do, especially when under stress,so is not a goof in the film.

Revealing mistakes

When Bridgette runs through the glass door, the glass breaks into fairly large pieces that lacerate her severely. However, when Jim is cleaning up the glass later, it appears to be pebble-like with some still dangling from the top of the door. Pebble-like glass fragments indicate that the glass is a tempered PVB laminated safety glass, which could not have injured Bridgette to the extent of her injuries.
When Linda empties the Lithium Carbonate 300mg pills into her hand, they are actually Acetaminophen 325mg pills.

Errors in geography

U.S. Route 57 is signed north-south, not east-west as portrayed in the movie, although most of the route is due east-west.

Plot holes

When Dr. Roth comes to take Linda away on the day of the funeral, her mother is heard asking her to tell them what happened to Bridgette's face. As the cuts occurred prior to Jim's death, she should already know what happened, although Linda would not because she hadn't lived through that day yet.
