When Steven comes around the truck to remove the gas pump hose, there is no hose going to the truck. They are all still in their holders on the pump.
When Steve is in a brutal fight with Burke at Burke's home, Steve crawls toward the fireplace and grabs a brass ash shovel and smacks Burke in the knee and head. But when he starts stabbing him with it, it becomes a black iron fireplace poker.
After Arthur's first hit in the movie, the (now deceased) victim is seen floating in the water, then suddenly, with help from Bishop, swimming forward. The camera pans underneath, showing Bishop's hands around the wrists of the corpse, but when seen from above, nothing is, as probably intended, visible.
When being chased on the roof, Steve McKenna shoots someone next to a gray door. He fires two shots but there are three bullet holes in the door.

After the first hit, when Arthur jumps into the river and grabs onto the barge to escape, he is clearly moving at the same speed as the barge before he even grabs it.
In the music credit for Schubert's Trio in E-flat (Andante con moto), 'Schubert' is misspelled as 'Scubert'.

At approx 17:45, Arthur is reading an article from the Cape Town Herald about the foiled assassination attempt. In the last sentence of the article, the first part of the statement says that "All five insurgents were stipped of any form of identification..." It should have said "stripped," not "stipped."
During the Cult Leader Assassination Job, the scene is supposed to be set in Chicago. There are shots of the Chicago skyline but the hotel where the action is set was in fact shot in New Orleans. Quick shots of palm trees, external building details and skyline (when the characters are fighting on the rooftop) gives the illusion away.
Although Arthur is supposed to be meticulous, he leaves a highly incriminating gun in a place where it can be found by someone who isn't even making a search.
Hotel room of the Cult Leader is equipped by design with two way mirrors. It may be quite unusual even in Chicago. :-)
While Bishop is exchanging money with the gun dealer when McKenna tags along on a hit, mike is clearly visible for several seconds in the top center frame.
