The Butterfly Effect 2

Continuity

When Nick is in the hospital after the first crash, he goes into a convulsion and the nurse runs in and hits the button and announces a "code blue". That should get every available doctor and nurse in the area to run into the room. Then the mother says he's alright now, yet the nurse never calls to reverse the code blue, and no-one shows up.
In the scene when the Jeep blows the tire, the camera angle clearly shows the right front tire as being flat. The next shot with the sparks, they are coming from behind the camera. In the shot showing the Jeep spinning around, all four tires are inflated.
Before the title sequence, when the tire of the Jeep explodes it sends the Jeep into a skid. We see a semi trailer approaching, and the driver of the truck applies the brakes hard enough to cause the trailer to jack-knife (this is shown quite clearly and dramatically). In immediately subsequent shots, however, the trailer is shown braking straight and true and there is no evidence to suggest that it was sliding sideways, but it is impossible to recover from a skidding jack-knife without stopping completely.
During the jeep crash, in one shot the jeep is being pushed to the left and in the next to the right.
After the truck hits the jeep near the beginning of the movie...the jeep is pushed out of its way twice in succession. No separate camera angles are obvious.

Revealing mistakes

When Julie is taking photos before the Jeep crashes, we can easily see that the camera lens is shut.

Miscellaneous

Nick locks his car in the garage even though it's a convertible with the top down.
