When Det Poole is shot outside of the crack house, the window from which he is shot is located to his right. When he is shot, he clutches his hand to the left side of his neck. There are no visible wounds to the right side of his neck, where they should be.
When Patrick, Angie and Helene McCready are driving in a car, they don't have their seat belts fastened. Then for a moment Patrick has it, and then again he doesn't.
When a police officer compliments Patrick on the way he handle the events at the quarry, the officer says "...the other night..", but it's been previously established that this scene is occurring two months after Amanda's disappearance.

Bressant says he and his partner went to a "crack den" in the Columbia Point (Housing Project) in 1995. In reality, by 1995, Columbia Point had for roughly 5 years, been rehabbed as "Harbor Point", a high-end mixed-income development. The kind of crack den that Bressant says he broke into had pretty much been closed down.

The same extra is seen at the very beginning of the movie and towards the end of the movie. She is standing on a porch with the same midriff shirt and same hairstyle in both scenes. Not only is this not a mistake, but in fact it seems intended. The scene at the beginning of the movie showing the neighborhood actually does occur chronologically at the end.

When Helene is telling Patrick about her date, she says "He drove down here from Providence." Boston is north of Providence, so she would have said "up here."

When Captain Boyle first meets Patrick at the McCready house on day 3, Boyle says "it's 76 hours and counting". 76 hours is 3 full days plus 4 hours, so counting from the kidnapping at 8:30pm this scene would be taking place at 12:30am, but the scene takes place in daylight.
