During the scene where Mike unplugs the clock radio from the outlet, the liquor bottle that he was given disappears from the table for a few shots, then eventually returns.
When Mike Enslin enters 1408, he flicks on the first two light switches nearest to him. The camera cuts away and when it cuts back, only the middle switch is flicked down.
After being in the heating ducts, when Mike falls out of the ceiling grate he is still soaking wet but when he stands up, immediately after, he is almost dry.
The amount of blood on the door from Mike's left hand changes between shots.
Mike looks down the street several times. At 00:38:49 and at 00:42:33 the same three cars are moving slowly on the road.
When the fire sprinkler goes off, Mike grabs a piece of wood and is shown hitting the sprinkler head to break it off, and then the water stops flowing. That is not how fire sprinklers work. Once they activate, the only way to stop them is to turn off their water supply.
During the uncut version of 1408, Mike turns on the TV set in his hotel room and immediately has access to pornographic titles like "Housewife Orgy" and "Wild Cherries Unleashed." This is extremely unusual since most American hotels restrict access to such content to keep young children staying in the rooms from accidentally viewing content that's graphic in nature. Even if the Dolphin Hotel were considered an exception to this, it's highly unlikely that the films would be given blatantly sexual titles because even if the films were restricted to adults only, children could still read the titles. Hotels usually have a password that unlocks such content and it's given discreetly to whomever booked the room. This way they don't have to be liable for children who view content that could be deemed offensive or vulgar. Many times gory horror films, films with racial slurs, films with nudity and films with profanity are also restricted via password.
After Mike Enslin calls for the engineer, he starts dictating into his micro recorder. In the background you can see his bedroom clock which reads "20:03". This is because the movie was filmed in London. In addition, later in you can see the clock says "8:07"
When Mike Enslin is on the ledge outside, the size of the ledge changes. When there is a shot of just his feet, he appears to only be able to fit half of his foot on the ledge at most, however when there are long shots which include Enslin's feet, they are fully on the ledge and are given about an inch of space. However, as the movie's main theme is based around reality and Mike's (and therefore our) perception of it, this may well be deliberate - the ledge really is wide enough for his feet but to his eyes it's far too narrow, thus heightening his fear.
If no electronic gadgets can work in the room how does Mike's laptop work? The room is meant to mess with Enslin's mind, so it would be guessed that letting him use these items are for mere amusement of the room.
While Mike Enslin is lying on the floor, his hand goes in and out of the bed comforter repeatedly. However, as this happens with each change of shot and is a very obvious inconsistency, it's almost certainly deliberate and done for thematic effect. Especially as, moments later, he keeps changing between lying down and being propped up in between shots, apparently without moving.
If the room receives a "light turn" once every month, why are there bloodstains showing up on the sheets under the UV light? However, that's just the room messing with Mike.
The door's hinges to room 1408 in the Dolphin Hotel are on the left when seen from the inside. Shortly after the engineer leaves, Mike closes the door and the hinges are seen on the right side of the door. Both seen around the 0:39:00 mark.
When Mike is clutching the brick wall, his nose presses into the "brick" and it gives way, revealing that it is fake. His hands also cause the brick wall to rustle on multiple occasions.
When Mike contacts his wife using his laptop, it is clear that he's using Windows XP (blue taskbar with the green Start button), however when the sprinklers go off, the well-known Blue Screen Of Death is of the Windows 98 type: 'Windows' in white on grey background, the rest of the screen being blue, and the error message centered on the screen. The XP BSOD type is simply a little darker blue with white text aligned to the left.
When Mike sees him hang himself with bed sheets in the mirror, it's very obvious that the person (a stunt double), is wearing a harness, as the the sheets are centered several inches behind his neck even when he bounces and comes to a stop. It's also apparent that a bungee was used since the person in the mirror bounces back up about 2 feet.
When Mike Enslin sees himself and his wife in the hospital discussing their daughter's illness, it is obviously his double sitting in the chair.
When Mike puts the Bible down on the table, the title "Holy Bible" is backwards, indicating that the image was "flipped."
A child's coffin is shown burning and is meant to be his daughter being cremated, however later we see his daughter has been buried in a cemetery.
When giving the "Skull Rating" of the first hotel he stays at, he initially gives it "six skulls." He then says "screw 'em, five skulls," but his lips say "fuck 'em, five skulls."
When Enslin runs towards the door and finds it locked, a lot of the sound effects seem a little off. For instance, when he grabs the doorknob, the sound of him grabbing it occurs a little before he even touches it.
The Dolphin Hotel is supposed to be at 2254 Lexington Avenue. Later on in the movie it is mentioned to be at the intersection with 45th Street. 2254 Lex Ave, however, is near the intersection with 84th street.
Dolphin Hotel manager Gerald Olin tells writer Mike Enslin that he could, "...take photographs in 1404, it has the exact same layout [as 1408]..."
The evacuation plan on the back of the hotel room's door shows that the layout of 1404 is nothing like 1408. Additionally, the plan shows that Enslin's walk from the elevators to his room is full of errors, in both the signage (the number of rooms on that half of the building should not go as high as 1430) and position of rooms along his path (he makes too many right-angle turns and there is no elevator just outside 1408).
Farrell mispronounces the name of famous artist H.R. Giger. The correct pronunciation of his last name is Gee-ger.
