They're in South Africa where traffic drives on the left side of the road. Yet Maxine is standing on right side of the street while waiting to be picked up for dinner, and while Bulldog is driving John and Maxine to the restaurant it's clear they're driving on a street with two-way traffic using the right side of the road.
John Seeger and Maxine Barnol infiltrate the bank by telling a security guard that they are with the Cape Town (South African) police department, but when the real CT police arrive, the guard refers to them as "the Americans" without any further input.

Factual errors

John Seeger's truck, which is registered in Arizona, has a front license plate. Vehicles in Arizona are issued one license plate.
Several vehicles parked on the street have front license plates. Vehicles in Florida are only issued one license plate.

Revealing mistakes

In the fancy restaurant, the cellist's bow is clearly not touching the strings of her cello when she is 'playing' it.
The police-vehicle sirens in S.A. don't sound like those used in the film.
After the rest room fight when Bulldog walks in and the camera pans around at all the destruction, you can hear a bolt being pulled from behind the wall, and see the wall move, as a urinal comes loose and falls to the floor.

Miscellaneous

Near the beginning after they have just driven through the building shooting people, the building seems to explode for no apparent reason, no explosives were left in the building, and the packaging in the boxes were just bits of shredded paper!
Chapel's title card as a "Black Opps Producer" is misspelled. It should be "Ops".

Audio/visual unsynchronised

In the restroom fight, several of John's punches clearly miss, yet there is a punch sound.

Errors in geography

There is no Dade County in Florida. Miami is located in Miami-Dade County.

Character error

At the end of the movie, flowers can be seen on top of the flag draped casket. Nothing should ever be placed on top of the U.S. Flag. Reference "Etiquette of the Stars and Stripes, Public Law 94-344.
At 42:42, after Dresham and Maxine meet Chief of Police Malik at the Credit Natal Bank in Cape Town, Dresham hands Malik a CIA/State Department memo. In the ATT: field, the country is listed as "South African" not "South Africa". In the body of the memo, the word "States" is omitted ("United Central Intelligence Agency"), "request" should be "requests", and the word "there" is incorrectly used for "their" ("our agents in THERE investigations").
