Continuity

When Sarah and Nelson return from the first oncology appointment they park the Porshe in front of the left of the garage. When Lucy and Sophia pull up later, the Porshe is on the right of the garage.
The scene in Carter's bedroom you see Lucy sitting on the bed in the mirror. As the shots change, the position of her hair and legs change several times.
When Carter and Lucy come home from the party the drive way is empty. However when Carter steps out of the car you can see a part of Sarah's SUV.
Carter and Sarah are taking a walk and then decide to go to the grocery store. Sarah's necklace changes between scenes even though she never goes inside to change...her outfit stays the same.
When Carter and Lucy are talking on the football field, Lucy's car has only the parking lights on. A few shots later, both the parking and normal headlights are turned on.

Factual errors

When someone is suffering from acute neutropenia (a common low white blood count following chemotherapy), as is the stated case after Sarah is rushed to the hospital upon collapsing, she would be kept in an isolated environment and her family would not be free to just run into her room off the street and collapse onto her bed, hugging her. At the very least, they'd be gloved, gowned and masked to guard against the spread of infection to the patient.
In the end credits for the soundtrack they title a song wrong: "Better Never Than Late" by Two Hours Traffic is actually "Better Sorry Than Safe". The official soundtrack has this corrected.

Revealing mistakes

The first time we see Sarah in the hospital after she cut off all of her hair due to treatment, we can see her hair right as she is putting on her cap.

Errors in geography

The scene when Carter is talking to Sarah one last time, there are fallen fall leaves blowing around. However, as the camera pans around the landscape, all of the trees and bushes are shown to be green and show no signs of the season being fall.
In the scene where Paige and Sarah are having an ice cream together, signs from nearby stores are faintly visible near the top of the screen. Near the end of the scene, the logo for "Canadian Tire" is clearly visible. Canadian Tire only has locations in Canada, even though the scene is set in Michigan.
