Continuity

The first time they get in the car, Bree insists that Toby wear his seat-belt. A few shots later, she's clearly not wearing her seat-belt while driving. In the midst of a conversation, the seat-belt is fastened.
As Toby is packing his backpack, he pulls out the dog encyclopedia and puts it on top, but a moment later it is gone, then back again.
When Toby shows up at Bree's apartment and is smoking, the plate Bree gives him to use as an ashtray moves around between shots.
In the final scene, the plate being used as an ashtray changes position, and the amount of ash fluctuates on almost every shot
When Bree is relieving herself behind the car, she pulls a girdle down on her thighs. When she gets scared by a noise and stands up, the girdle has disappeared entirely.

Factual errors

When Bree is shown coming around after having had her final surgery, her fingernails still have polish on them. Nail polish is always removed prior to surgery, so that any changes in coloring (nails turning blue due to lack of oxygen) are detected by medical personnel.
Bree is on hormones up until the day of her surgery. In reality, estrogen can't be taken for several weeks beforehand because it increases the risk of blood clots.
The bottle of Bree's mother's Premarin that Bree takes from the medicine cabinet is labeled as .06mg. Premarin is not available in that strength. It is available in several strengths, including 0.625mg. A 0.06mg dose would be sub-therapeutic.

Revealing mistakes

The desk sergeant's name tag is blank.

Audio/visual unsynchronised

When Bree is watching Calvin play a song on his guitar, his fingers never touch the guitar, the strings don't move at all.

Crew or equipment visible

In the wide shot of the first scene in the car, as Brie tells Toby to "take his feet off the dash," the camera truck can be seen in the reflection on their car.
In the wigwam scene, crew members and the white screen reflector are visible in Bree's sunglasses.
At the Trans-pride party when Toby holds Bree's dress in front of himself at the mirror and Bree comes out of the bathroom, a crew member in the bathroom is visible through the crack in the bathroom door.

Errors in geography

During the trip from New York to California, Toby asks why they are heading to Dallas. The scene changes from a Midwest setting to inside the Deck Park Tunnel on I-10 in Phoenix heading West (already past Dallas), back to a Midwest neighborhood.
Bree talks about not going west on I-95. She couldn't in any case--I-95 runs north-south.

Character error

Bree's sister, half Jewish, misuses the Yiddish phrase "kin-ahora" when she hears about Toby's mother's suicide. The phrase means "may the evil eye be averted" and is roughly equivalent to "knock on wood". It is used when you say something GOOD, to avert a spell cast by a jealous person or a demon. It would NEVER be used with respect to something bad.
