When Neelix shows B'Elanna around her quarters, trying to help her remember, the bat'leth hanging on the wall is not the one given to her by the Klingons in a previous episode. The one hanging resembles Worf's bat'leth.
You can see Voyager coming about before the ECH issues a command to do so.
ECH was missing his Captain's pips.

Incorrectly regarded as goofs

Kim says that he "didn't see Neelix insist that Chakotay" try the delicacy that is making Kim sick. Neelix says that Chakotay is a vegetarian. Kim says that it was made from meat, and Neelix corrects him, saying that it is made from meat byproducts, which appears to be an error by Neelix. Vegetarians are commonly assumed to refrain only from meat, not from meat byproducts, but in fact, vegetarians commonly avoid meat byproducts as well, whether they self-identify as vegans or not.
