When Louis takes Joe to his Alphabet City (tenement) apartment, he opens his door which is in a long line of doors down the hallway. Once inside, he suddenly has two large windows, front and back, where there shouldn't be windows because there are more apartments on either side of his.
When Lou is saying kaddish for the dead Roy Cohn, Roy's mouth goes from open to closed to open again.
When Belize pulls Roy away from Joe in the hospital hallway, Roy's hand jumps from Joe's shirt to down at his side between shots.
When Belize puts the "goop" on Prior, in one shot Prior's hospital gown is off his shoulder, in the next, its completely on.

Factual errors

Roy Cohn is depicted as dying in a New York hospital. In real life, Cohn died in Bethesda, Maryland.
In the film's final tableau, Prior Walter (Justin Kirk) describes the Bethesda Fountain (Central Park, New York City) as his favorite place in the world, pointing to the famous 1873 statue behind him saying, "This is the Angel Bethesda." This is not quite accurate. The Fountain was inspired by the legendary pool mentioned in John 5:2-4 which was called by its Hebrew name, Bethesda. The Angel, mentioned in Verse 4, is not named, and has no gender.
When the mourner's kaddish is recited at the bedside of Roy Cohn, a part of the kaddish is omitted, most likely in error. It's only a few words, in the middle of the kaddish, whose literal translation is the phrase: "beyond all the blessings and hymns."

Revealing mistakes

In the scene between Joe and Hanna at the Mormon visitors center while the 2 characters are arguing it's supposed to be raining outside, but if you look close you can see sunshine in the background and a woman walking in shorts.

Anachronisms

The movie takes place in 1985, yet the funeral in Perestroika shows a drag queen dressed as a Barbra Streisand circa 1993.
When Hannah is trying to hail a cab for Prior outside the Mormon Visitors' Center, the taxis that drive past them are from the 1980s but the clearly visible cabs in the background are from the 1990s/2000s.
The Coke can from which Joe drinks in the 1985 time setting (outside Justice with Lou) has the label "Coca-Cola Classic", rather than "Coke" or "New Coke" (introduced on 23 April that year). The design of the can is from 2002, not 1985.
The Doritos bag Harper has in the Mormon Visitors Center didn't exist in 1985.
The NYC subways and its signs were modern rather than from the 1980s.

Audio/visual unsynchronised

When Prior waits for Louis to approach in the park the music playing is '90s hip-hop, not mid-'80s rap.

Errors in geography

Hannah arrives at Clark St station, but when she walks along the platform a sign for 42nd St, where the scene was presumably filmed, can be seen.

Character error

Prior says that "Thomas Jefferson died on his birthday". Jefferson died on 4 July 1826 but was born on 13 April 1743.
