When Graciela removes a dish from the oven, she uses oven mitts, presumably because it's hot. It's clearly Tupperware, however, containing celery sticks and carrots. McCaleb even briefly rests his knuckles on the lid a few minutes later.

After Terry discovers that the killer takes mementos from his victims, the 'blowup' of the killer taking Gloria's earring is taken from a completely different angle than the previously seen surveillance tape (not to mention so clear that the other detectives could never have overlooked it).

When Terry is paying Buddy, he asks Buddy who he should make the check out to, Buddy says "Jasper Noone"; later when Terry brings the check to Buddy the check is made out to Buddy Noone.

As Arrango and Waller are floundering in the water at night, Arrango tells McCaleb he needs to come to a crime scene right away. However, when they arrive it is bright daylight and Arrango and Waller are both dry, indicating that they took several hours to get to the scene.

When Jasper Noone dives into the oily water on the wrecked ship, bullets hitting the water stir up mud and such. Afterwards, Noone doesn't have a speck of oil, mud, or any other type of filth on him.

When Dr. Fox is doing her blood work on Terry, an x-ray is used whilst performing the biopsy. Both times, neither she nor anyone around are wearing protective lead garments against radiation exposure, which is a standard practice in any medical establishment. In addition, an endomyocardial biopsy, as it is called, is not preformed in a medical office as shown in the movie. It is performed in a cardiac cath lab, which is effectively, an operating room. All personnel are in scrubs and a sterile field is required. Lead is worn underneath the surgical gowns. From an outside perspective, it may not be readily apparent that the staff in a cath lab are wearing lead IF they are wearing surgical gowns appropriately.

Several times reference is made to McCaleb having bloodtype AB with CMV-. For transplant patients, CMV means nothing. Of much greater importance would be whether they were both rH- as someone who is rH- would develop antibodies and quickly reject tissue that was rH+.

On the tape from the ATM security camera, the victim looks to his left just before he is shot in the head. While his head is turned you can see the wire for the blood pack running down the back of his neck.

When McCaleb is doing a newspaper search on a computer, both search results are labeled as "1 of 1" and the result ID is the same, even though the headline and article summary is different for each result.

In the article about the cemetery killer on Det. Winston's wall, the word "heinous" is misspelled "hanus".

One of the detectives calls the Stanford University team the "Cardinals". They are actually the "Cardinal" (color not bird). The name was changed from "Cardinals" to "Cardinal" on November 17, 1981, by university president Donald Kennedy.
