The scar on Nick's face switches from cheek to cheek throughout the last half of the movie.

Factual errors

There are two scenes where helicopters are used: in Cambodia when 'Vietnamese' helicopters pass overhead and on the Chechnyan border. Both of these scenes were US-made Bell helicopters, the UH-1 Huey. It would be more factual to use Russian-made helicopters in these instances, particularly in Cambodia.
A Khmer Rouge officer claims he only answers to god, when in fact the Khmer Rouge brutally surpressed everyone who did not abandon their religion. The Khmer Rouge believed in only one thing, and that was 'Angkar', the ruling body of the Khmer Rouge.
Dr. Nick Callahan claims a banana has 300 calories. In fact, a medium banana usually does not contain more than 80-100 calories when peeled.
The scene with the baby and the grenade has rebels using a pineapple grenade which has a rough kill radius of 5 meters and 15 meters for maiming/wounding.

The rebels appear knowledgeable of their weapons and the "leader" ignores the fact that they would all die if the baby pulled the pin out.
In the scene where the rebels put a grenade next to the baby, they put a pineapple grenade in its lap. The pins on a grenade has a cotter pin with the ends bent to prevent accidental explosions. The rebel does not take the time to straighten the pins making it impossible for the child to pull the pin.

Revealing mistakes

At 1:03 when Sarah arrives in Cambodia, there is a distinct dolphin cry in an area that appears to be land locked.

Miscellaneous

Jimmy Bauford is 4 years old in the 1989 segment and 10 years old in the 1995 segment. He is played by the same child actor in both segments, and he does not age a day.

Anachronisms

When the police van drives the Ethiopian boy to the airport in England, they drive past an Air Transat jet. The scene takes place in 1984, while Air Transat made its inaugural flight on 14 November 1987.

Errors in geography

Sarah says that Namibia is on the Ivory Coast of Africa, when it actually is on the Skeleton Coast.
