The letterman's jacket in Jake's room is spelled "Lions". Ben takes it from his room. When Christopher wears it at the hospital, it is now spelled "Lyons".

When Madison and Ben are talking on-line, her screen name is clearly stated to be "SwimFan85". When she signs off, Ben's computer says "Swimfan has signed off".

When Ben and Madison are hooking up for the first time in the pool, Madison's hair is dry. However, the camera cuts to another shot of them from a different angle and Madison's hair is suddenly wet even though she never dipped her head under the water.

Ben takes off his jacket when he arrives at Amy's party. When Amy introduces him to Madison, he's wearing the jacket for one shot.

When Madison kisses Josh in the pool, there's a close up on Ben and a man is walking behind him. In the next shot, the same man walks past again.

Numerous mistakes related to competitive swimming - there are no 25 yard races after age 10. The shortest high school race is the 50 freestyle, a sprinter's event that would be accompanied by the 100 freestyle.

When Ben bumped his head into the dead body of Josh, he should have seen him in the pool when he dived in.

When Madison sends Ben a picture of herself, a cloth can be seen around her side behind her right arm used to cover up her cleavage

The second time Ben brings in medication for his elderly patient friend (shortly before he flatlines), we see Ben's medicine cart. All the pills in all the cups are exactly the same.

When Ben goes out on the porch to see the police car, the sound from his steps are of hard sole shoes, After he wakes up from his conk on the head it can be seen he's not wearing shoes.

When Madison is playing the cello, her fingerings, the vibrato, and the bow strokes are not in time with the music.

The camera is reflected in Ben's truck when he is driving.

When Ben and Madison are in the pool the first time, at one point there is a hand on Ben's right shoulder. It is not Ben's nor Madison's hand.

Madison uses Ben's vehicle to attack Amy assuming people will think it was Ben. Ben is not even in the same city when this happens and is in New York. He would've been seen on camera at the hospital, and considering so many staff saw Ben he would have a solid alibi.
