A helicopter brings Claire to Rockfort Island as a prisoner, where she is knocked out by a guard. We go back ten days, when Claire was caught trespassing in an Umbrella facility in Paris in search of Chris. She is chased by a helicopter and many armed men, but manages to outrun and outgun them. However, she is soon held at gunpoint by a guard. Back in present day, Claire wakes up in a cell to the sound of falling bombs. The guard who took her hostage earlier, called Rodrigo, comes in badly wounded, and is without hemostatic medication. He opens Claire's cell and lets her go, saying that a special forces team has taken over the island and wiped out their own troops, so she might as well go free. However, he tells her that there is no chance of getting off the island.
Outside, there are fires everywhere, even a crashed helicopter. Claire discovers that the T-virus has broken out, since zombies are everywhere, even coming out of fresh graves in the ground. She is suddenly fired upon from a guard tower, but it turns out to be a fellow inmate called Steve Burnside, a young man who mistook her for a zombie. Steve says that there is an airport on the island that he plans to use to escape. Claire wants to team up, but Steve prefers to work alone, saying that she will only slow him down.
Claire investigates the prison complex, and finds a prisoner barracks. There is ample evidence there that the island is led by a man called Alfred, who allows the prisoners to be used in all kinds of inhuman experiments. In the guard station, she finds Steve accessing a computer, learning that Umbrella has put Claire's brother Chris under surveillance. Claire uses the computer to contact Leon and give him her coordinates, so that he can pass them on to Chris. Steve gets agitated, telling her that she shouldn't rely on others for escaping because she will only end up disappointed, and leaves.
Claire finds a way out of the prison complex, and crosses a large bridge to the other side of the island, which has a large palace-like mansion. From documents, she learns that Alfred is an unstable man who lives on the island with his beautiful sister Alexia, although she is rarely seen as he is extremely protective of her. In Alfred's office, she sees a video of Alfred and Alexia as kids, and they do seem particularly close. In the lobby, Claire has a run-in with the real Alfred and his hunting rifle. Alfred, a member of the noble Ashford family and also the grandson of one of Umbrella's founders, is enraged that his base has been destroyed, which caused the release of the T-virus that created countless zombies and monsters. He seems to be under the delusional idea that Claire allowed herself to be captured by Umbrella, so that her people could follow her to the island and attack it. Claire denies everything, so he will let her live and toy with her a little more.
Claire locates the airfield, but the planes are only accessible with special keys. She finds a Military Training Facility next to the palace with a laboratory and a cleanroom. Looking through a window inside the room, a scientist begs to be let out, but he is suddenly grabbed by a yellow claw that crushes his head against the window. This sets off the biohazard alarm, causing automatic shutters to seal off the surrounding area; Claire barely manages to get out before she is sealed inside. In the facility's courtyard, she is again ambushed by Alfred and his rifle. She flees back into the building and finds a hemostat for Rodrigo, but suddenly, Alfred seals another shutter, trapping her in an area full of monsters. Via the PA system, he challenges her to try out his "special playground" and stay alive.
The monsters in question appear to be hideous yellow Tyrant-like creatures with an extendable arm. Claire is suddenly ambushed by one that nearly kills her, but then Steve enters and dispatches the monster. Alfred, who kept watch, activates a large floor elevator that sends them into the basement, telling them that Steve can join Claire in death. However, with the help of a few automatic weapons, Steve quickly clears the entire floor of all zombies. He shows his trust issues again when claiming that guns are more reliable than people, so Claire asks him about his reasons for being on the island as well as his family. Steve gets angry and says that he doesn't want to talk about it. In the next room, the floor suddenly collapses, and Claire's foot gets trapped beneath the rubble. Claire is menaced by a zombie, but Steve is unable to take action, despite Claire's calls for help. Finally, he kills the zombie with a howl of despair, saving Claire. The zombie turned out to be Steve's infected father, who was an Umbrella employee who foolishly thought he could steal corporate information and sell it to the highest bidder. However, he was caught, his wife killed, and both he and Steve were sent off to the prison island.
Claire leaves Steve to recoup from his ordeal, and finds a command center inside the facility. On a screen, she sees that the biohazard alarm has stopped. She enters the lab in the hope of finding more keys, but inadvertently releases an immature Albinoid creature. It escapes through a grate and the biohazard alarm is immediately reactivated, so Claire has to run again not to get locked inside. She returns to the palace, where she learns that the Ashford dynasty was started by Veronica Ashford, a woman of legendary intelligence and beauty. In Alfred's office, Claire discovers a secret accessway to a covered bridge that leads to a private residence away from the facility. Inside this mansion, she overhears a conversation inside one of the bedrooms, where Alexia urges Alfred to get rid of Claire, as the revival of their family depends on his success. Alfred seems totally devoted to Alexia, and intends to make her head of the family. Claire needs to leave to prevent being spotted by Alexia.
She returns to the prison and gives Rodrigo the hemostat, saving his life. For saving her life, she also gives him her lighter, once a present from Chris. But when she returns to the palace, she runs into Albert Wesker, who admits that he attacked the island and wants to take revenge on Chris, who will certainly show up now that Claire is there. He violently beats her around with superhuman strength and glowing red eyes, ready to kill her, but he is suddenly called away by his men, and leaves while saying that she may be of further use. Claire proceeds to the private mansion, where she finds a confession letter from Alfred. He clearly has an unhealthy obsession with his sister, who is a scientific genius, and wishes for her to rule the world. Suddenly, Alexia appears with a gun, ready to kill Claire, but Steve intervenes and manages to wound Alexia, who flees through a secret door. Steve and Claire follow, and they find blood as well as a blond wig. Alfred suddenly attacks, but Steve fends him off. Alfred sees himself in a mirror, wearing his sister's make-up, and runs away screaming in anguish. Apparently, there is no Alexia: Alfred cross-dresses as his sister at times, switching between her personality as well as his own.
But then, the PA system announces that Alfred has activated the self-destruct system. Steve and Claire see airplanes with other survivors flying over, so they go to the airfield, but a lowered bridge prevents Steve from taking off. Claire raises the bridge, but while returning to the plane, Alfred is at the command center, still speaking in Alexia's voice, getting back at Claire by releasing a T-078 Tyrant from one of the labs. The Tyrant breaks out and blocks Claire's path, but she defeats it and boards the plane. The plane takes off and Steve apologizes for causing so much trouble, hoping that she will meet her brother again. In the meanwhile, Alfred stumbles to a secret passage behind the training facility, opens a jet hangar and takes a Harrier jet in pursuit of Steve and Claire. In the plane, Steve hears a loud sound and notices that the cargo room hatch is open. Claire investigates, seeing that the Tyrant has stowed away on board. It is a difficult fight in a cramped area, but she finally ejects the monster from the plane with the cargo catapult.
The relief only lasts a short while though, as Alfred hacks into the system and maliciously announces that he has set their plane on an autopilot course. With no other options, Steve and Claire take a nap. Steve develops romantic feelings for Claire, and almost attempts to kiss her. As they see the other planes parked on the ground, they learn that their destination is Antarctica. Their plane makes an emergency landing, crashing into another Umbrella facility building. With the plane completely trashed, they decide to split up and look for a way out of the base.
This facility has also suffered an outbreak, and is crawling with infected zombies and mutated monsters. After restoring the power, Claire finds a memo of Alexander Ashford, father of Alfred and Alexia, detailing how his father Edward developed the T-virus from the mother virus found by Lord Spencer, and together they created Umbrella as a front for their illegal research. However, when Edward died, Alexander, who majored in biogenetics, was unable to continue his father's virus research, and his family's good name dwindled. Fearing that Spencer's competing branch of the company would take over, Alexander secretly built the Antarctica base, hoping to expedite his research, restore the Ashford name and "revive Veronica". Claire then learns that Alexander disappeared unexpectedly 15 years ago, and soon after, Alexia died during an experiment gone wrong; the sudden loss of his father and sister nearly caused Alfred to go mad. Claire then finds a basement cell, holding a grotesquely mutated humanoid called the 'Nosferatu'. She rejoins Steve, who has learned of an Australian observation base seven miles away. They plan to use a digging vehicle to cut through the wall, but while prepping it, Steve gets distracted by Claire's beauty, and inadvertently breaks a pipe, which fills the room with toxic gas. With no other option, they split up again, looking for a way to shut down the gas.
Claire eventually succeeds, but she is then held at gunpoint by Alfred, still talking as Alexia. Again, Steve comes bursting in, and in the ensuing gunfight, Alfred is shot several times, staggers and falls down a deep chasm. A roar is heard as the Nosferatu escapes from its cell. Steve and Claire use a digger to break through the ice. Water floods the basement levels of the base as they dig out, and proceed on foot to a raised platform. However, the Nosferatu then appears, pushing Steve nearly off the platform and attacking Claire. She defeats it, but cannot avoid being poisoned by the creature's toxic fumes. She saves Steve, who vows that he will keep her safe from now on. They proceed to a snowcat and drive away, but in the meanwhile, Alfred has dragged himself into a lab, vowing revenge on Claire in his own voice. Mortally wounded, he crawls to a cylinder containing his sister Alexia, just as the devices around them come to life and the cylinder is drained of water. She steps out just as Alfred calls out her name one final time, and dies. As Claire and Steve flee across the ice, believing they are safe, a giant tentacle sprouts from the base and overtakes them, grabbing the snowcat and violently smashing it to the ground. Alexia watches the burning wreckage of the snowcat on a monitor as she cradles Alfred's dead body...
In the meanwhile, Chris Redfield has arrived by boat on Rockfort Island, following a lead given to him by Leon. He climbs a rockface and ends up in a series of catacombs, finding a wounded Rodrigo. He tells Chris that Claire probably escaped the island on one of the planes, and urges him to find a way to follow her. Suddenly, Rodrigo is eaten by a large worm coming out of the ground, who swallows him whole. Chris manages to kill the worm, who spits out Rodrigo. Thankful that he may see his family again, he returns Claire's lighter to Chris, and dies peacefully. Chris takes an elevator and finds himself in the training facility, discovering Alfred's hidden jet hangar, but it is unfortunately locked. He also locates the command center, which shows Alexia on the large screen as she holds the deceased Alfred and sings him a lullaby. Near the airfield, Wesker sees the same broadcast, and is dismayed that Alexia is already awake. Seeing Chris on another monitor, Wesker frees a couple of Hunters, as a "welcoming gift" to keep Chris "entertained", as well as a small flying robot.
The island's buildings are heavily damaged by the self-destruct system, and may paths are blocked by rubble. Chris searches the training facility and labs, coming across Wesker's robot, which is actively searching for Chris and notifies the Hunters to his location. He flees to the courtyard, which has been cracked open by the explosion; Chris can see the key to the jet hangar falling and getting dragged down the sewer below. He learns that the sewer once housed an underground passage to Alfred's residence, but it got damaged, so Alfred had some prisoners built the covered bridge, and executed them afterwards to keep it a secret. While looking for a way into the sewer, Chris is surprised by a very much alive Wesker in one of the labs, who tells him that he has broken free of Umbrella's grasp, and sold his soul to another company to obtain Alexia. Realizing that Wesker attacked the island and his sister, Chris tries to fight him, but the superhuman Wesker knocks him to the ground effortlessly. While nearly choking Chris to death, he expresses his hatred toward him for destroying his plans. Chris manages to land a single punch, knocking away Wesker's shades and revealing a set of hideously yellow eyes. Wesker taunts him with the knowledge that Claire is in Antarctica with Alexia, when suddenly, Alexia appears on a monitor. Wesker throws Chris against a large cylinder and leaves, letting Chris fight a Bandersnatch that emerges from it. He finally finds a secret passage into the sewer, finding the hangar key in a pool guarded by the Albinoid, which has grown to mature size now. He kills it, obtains the key, opens the hangar and takes another Harrier to the Antarctic base.
Landing in a hangar bay, Chris finds the lower basement levels of the base frozen solid. In the office, he finds Alfred's diary, and it reveals a great deal: there is a sealed room in the base that can only be accessed with the three family jewels possessed by Alexander, Alfred and Alexia. Alfred got in, and learned an "insane secret" surrounding his and Alexia's birth, which was Alexander's attempt to restore the Ashford's good name. The siblings then carried out an experiment on their hated father, but it turned him into the uncontrollable Nosferatu. Alexia then decided to repeat the experiment on her own body, while kept asleep for 15 years. Missing his beloved sister for so long is what drove Alfred mad. Chris continues searching for Claire, spotting the scanner droid, signaling that Wesker has followed him to Antarctica. He eventually finds a mansion within the base, with an entry hall identical to the Spencer Mansion from Resident Evil (1996). He locates Claire, stuck to the wall and unconscious. He frees her, and after a happy reunion, he wants to take her away. However, Claire got separated from Steve after the monster attack, and she wants to save him first. Unfortunately, she collapses from the Nosferatu toxin, so Chris has to find an antidote first.
Chris ventures further into the lab, finding a huge silo with an enormous anthill, as well as the lab where Alexia awoke from her cryostasis. Alfred's corpse is inside her cylinder, and Chris takes Alfred's family ring. He also finds Alexia's masterplan for world domination: she was fascinated by ants, who are ruled by a single queen, and felt that due to her supreme intelligence, she deserved a similar status. She found remains of an ancient virus inside the genes of a queen ant, which she implanted into Spencer's mother virus. She tested this new "T-Veronica virus" on her father Alexander, but it transformed him into a brainless monster. She realized that to unlock the virus' true power, she had to slow it down. She implanted it into her own body, and went into cryostasis for 15 years, allowing the virus to gradually transform her into a queen, while guarded by her "inept but loyal soldier ant" Alfred. After her awakening, the virus will be spread over the entire world, and she will become its ruler. Alexia also mentions an antidote for the Nosferatu's toxin, so Chris continues his search. He finds a frozen pool in the basement, with the dead Alexander/Nosferatu hanging on a crane below the ice. He pulls the body up, but suddenly, Alexia appears, saying that she will dispose of insignificant bugs, and setting a huge spider on Chris. He defeats it and retrieves Alexander's family jewel, and locates the antidote with which he heals Claire. But suddenly, Alexia appears on top of the stairs with a taunting laugh; they chase her, but the giant tentacle appears again, destroying the stairs and wounding Chris. They hear Steve yell, so Chris urges Claire to go on without him and save Steve.
Claire finds a cell block and a keycard for the self-destruct system, which, upon activation, will open all locks. She finds Steve locked inside a long separate room, the same cell where Nosferatu used to be locked up. Steve says that the insane Alexia performed the same experiment on him that she did on her father. It turns out to be true, as Steve painfully transforms into a hideous monster. He no longer recognizes Claire, and chases her with a giant axe. Claire can barely outrun him, and just manages to get on the other side of a set of lowering bars that separate her from Steve, but the cell door is locked. To make matters worse, Steve breaks the bars with the axe, and the giant tentacle breaks through a wall and grabs Claire. Steve is just about to kill Claire, when he suddenly regains his memory, and cuts through the tentacle. It furiously releases Claire and mortally skewers Steve, who transforms back into his normal form. Claire tells Steve to hold on, but he is merely happy to hear that Chris has come and kept his promise, as Steve himself couldn't keep Claire safe. With his dying breath, he confesses his love for her, leaving Claire weeping and devastated.
Back in the entry hall, Chris is in hiding, watching how Wesker is confronting Alexia, and demands that she come with him because the only existing sample of the T-Veronica virus is now inside her body. Alexia laughs, telling him that he is not worthy of its power. She suddenly bursts in flames, mutating into a strange yet elegant creature, that proceeds to punch Wesker. Despite his superhuman talents, he is no match for Alexia's flammable blood and strength. When he sees Chris, he says that he will let him handle Alexia since he is one of his best men. Wesker leaves, and Alexia turns her attention to Chris. He shoots her and takes her necklace. Having obtained the three Ashford jewels, Chris enters the secret lab, not noticing that Alexia rises again after he leaves.
Chris finds a report, revealing the final Ashford secret: Alexander discovered the gene that governs intelligence in humans. In his attempt to revive his ancestor Veronica and elevate the Ashford family to glory again, he created a clone of Veronica with genetically-enhanced intelligence, and placed it inside a surrogate mother. Unexpectedly, the experiment led to the birth of twins: Alexia had unmatched intelligence, but Alfred's, though above average, was not enough to be considered a genius. Chris proceeds to the cell block and hears Claire crying over Steve in the cell. Since they are both unable to open the door, Claire shoves the keycard under the door. Chris uses it to activate the self-destruct system, freeing Claire. With the countdown running, they plan to leave, but find the giant tentacle blocking their escape. It releases Alexia, so Chris will distract her by attacking, while Claire will prepare a jet. However, Alexia transforms in an even bigger creature, and eventually becomes a dragonfly-like monster, but Chris finally charges an energy-based weapon that completely obliterates her.
Chris escapes, but finds Wesker holding Claire hostage. He explains that he no longer needs Alexia: his men have taken Steve's corpse which still contains the virus, so he can focus on his revenge on Chris. Chris starts battling Wesker head-on, while Claire rushes to to jet hangar. Wesker savagely beats Chris around, making the battle one-sided before Chris pulls a lever and drops a stack of iron beams upon Wesker. However, Wesker struggles to his feet and attempts to continue the battle, but Chris is saved when an explosion burns part of Wesker's face, and separates them by burning wildly in between. Wesker declares to continue their conflict, should they ever meet again, and Chris agrees. Chris flees while Wesker stands in the center of the fire, laughing maniacally.
Chris barely makes it to the jet as fires engulf the base, and he and Claire escape in the jet. Claire wants Chris to promise that he will never leave her again, but Chris declares that their battle with Umbrella is not over, as they fly off into the daytime sky and the base behind them blows up.
The main title comes on stating "Resident Evil- Code: Veronica X." The end credits show clips from scenes of the game before ending on a photograph of Claire and Chris going sightseeing.
