The "fake" blood on Benjamin's face disappears after filming the black guy getting shot multiple times.
The blue paint smeared on the actress's face disappears and reappears in between shots.
The amount of "blood" on the Razor Blade victim's hands.
The blood drooling from the dead black guy.
In between shots, the Toxic Avenger actor rolls out of the porta-potty, twice.

Factual errors

When a woman has an involuntary abortion, the preterm baby comes out crying. The baby would've already been dead, having just been forcibly cut from its mother.
'Soundtrack' is misspelled in the end credits as 'Sountrack'.

Revealing mistakes

Obvious usage of dummies for stunts.
During the opening credits when a man gets his leg ripped off, you can clearly see the pants torn. You can also specks of blood on his shirt and the leg stump from a previous take.
During the scene with the Mysterious Woman is given a pickup by a couple strangers, it's obvious that there is no scenery moving around in the driver's window.
When Toxie's arm flies off, you can clearly see a wire behind the stump used to pull it away (as the shot was done in reverse).
When the black guy gets shot in the head, a wire can seen pulling out the "head plug".

Crew or equipment visible

Camera shadows are often seen throughout the film.
After Jennifer passes a couple making out, a crew member's head is partially visible on the right side of the shot.
