(at around 10 mins) When Reggie begins to change the tire on the Barracuda, the convertible top is down. A few minutes later when he stands up, the top is up.
(at around 22 mins) After Reggie blows up the police car with the demon inside, the car door opens twice to show the demon escaping.
(at around 54 mins) Reggie tells Jennifer that he has been chasing the Tall Man 'for several years". However, the events of the past three films (Phantasm (1979), Phantasm II (1988), and Phantasm III: Lord of the Dead (1994)) have been portrayed as directly continuous leading back to Mike being released from the mental hospital in Phantasm II with the events of the subsequent films taking place over the course of only a few months.
Jennifer's custom Maverick has nice new tires & hubcaps, but after the car flips they are replaced by plain, cheaper ones.
(at around 17 mins) When Reggie is pulled over, he gives the cop his license and registration through the window. As he is waiting he can't see the policeman in his rear-view mirror. As he is getting ready to get out of the car to check, the window is still cracked about 6 inches. But when the scene cuts to him opening the door, the window is all of the way up.

Factual errors

At around 10 minutes in, when Reg is changing the car tires, he's twisting the bolts to the left, which in actuality would be loosening them. In the movie, however, it appears that he is tightening them.

Revealing mistakes

In the movie's trailer, there are a couple of scene inserts that features the video playback info on the top and bottom of the screen.
(at around 45 mins) When Mike wills the boulder to fall on the dwarf, a crew member's hand can be seen briefly in the upper left corner, presumably having just pushed the rock.
(at around 4 mins) Obvious plywood wall (not stone as it is made to appear) that Reggie interacts with at the beginning when he is confronted by the Tall Man.
(at around 16 mins) The red glow that is supposed to come from inside the coffin in the hearse appears a split second before Mike actually opens it.
Right before Jody appears in the graveyard, you can already see him at the tree in the background with Mike. Then Jody pops in at the fork gate, and only Mike is at the tree. Apparently they used footage from later in the scene for before the splice.

Crew or equipment visible

At about 58:40 when Mike looks Thru the glove compartment of the hearse and finds a knife, at the lower left of the screen you can see the reflection of someone wearing shorts in the black paint. It is not Jody as he Is near the front of the hearse and the passenger door is open thus blocking him? Plus, Jody is not wearing shorts.

Plot holes

The film reveals that the Tall Man's incarnations are all copies of the original Jebediah Morningside, shown as a somewhat elderly man. However, in all the earlier films, the Tall Man is shown as much younger (to match the age of the actor who portrayed him) which contradicts information about him being a copy of the elderly Morningside.
