The Second Doctor notes that Jamie and Zoe had their memories erased when they left the Doctor. This was done by the Time Lords at the end of his last story, The War Games. But if the Doctor remembers this, why would the Time Lords have allowed the Doctor a brief vacation in the middle of his trial to visit the Brigadier's retirement party before forcing his regeneration?
When Sarah Jane is grabbed by the time scoop she drops her bag but her "figure" has a handbag over its shoulder.
When Sarah Jane Smith says to the Fifth Doctor, "It was really nice meeting you," it is the Third Doctor who responds, "Thank you, Sarah Jane. Nice meeting you, too," suggesting he was picked up from his time line before meeting her in The Time Warrior, while still recognizing her on sight earlier. His "I'll explain later," comment is a "hanging a lantern" on the Doctors' continuity errors of memory without having to actually explain them. While that can explain the Doctors' recall of what should be future events, it wouldn't explain the Brigadier's. Or he was just having a little fun confusing her by answering on behalf of his future self.
When the Master is transmatted from the Capitol to the Death Zone, he suddenly acquires a cape.
Before he knocks him out, the Brigadier says "Nice to see you again" to The Master. But the Brigadier has never met this incarnation of The Master before (played by Anthony Ainley) and is only familiar with his former incarnation played by Roger Delgado, so he shouldn't know who he is.

Factual errors

In the Special Edition commentary, it is mentioned that the "teeth and curl" comment from the Third Doctor was originally Sarah Jane Smith's line, but the actors decided to shift it for humorous effect. The commentary points out he shouldn't know of his future appearance and that shifting the line, while entertaining, was a continuity mistake. However, Sarah Jane Smith mimed pointing at her teeth and a twirl of finger at her hair just before it, allowing this act of charades to prompt the Third Doctor's line.

Incorrectly regarded as goofs

When Turlough and Susan are in the TARDIS, watching the view screen, Susan says "Cybermen!" when they see them approaching.

It may seem odd that Susan recognised the Cybermen as she never appeared in a story with them - however, she was with Tegan earlier in the story when Cybermen crept up on the Doctor and Tegan called out their name as a warning.
Of all the actors who portrayed the Doctor, Jon Pertwee had visibly aged the most with solid white hair and looking many years older from his last appearance in Planet of the Spiders: Part Six (1974).

Although this makes little story sense since the Third Doctor would have been taken out of time and should have looked the same as during his years at UNIT, it's unavoidable from a production standpoint given that Jon Pertwee was nine years older than when he had last played the role.

Revealing mistakes

As the Brigadier frees his arm from the grip of the Cyberman reaching through the gap in the wall, it can be seen that the actor playing the Cyberman is wearing denim jeans and not a full costume.
In the Special Edition, the departure of the legacy characters is changed from departing in dematerialized ghost images of the TARDIS to the updated "Casper" or "Mr. Whippy" depiction of the Time Scoop being used to return them. Not only was it depicted earlier that the Time Scoop could not reach the Doctor inside the TARDIS, but the Doctor's next line, "Temporal fission. Old Rassilon is very clever," referred to the removed effect of splitting the TARDIS and doesn't make sense with using the Time Scoop. This is compounded by that line not being used in the original version, so it was brought in into an altered context.

Miscellaneous

When the Master warns the First Doctor and Tegan Jovanka of the imminent arrival of his unfriendly associates, the Cyberleader turns the corner and reacts to the room while looking directly at the Doctor before he and Tegan manage to hide, his view unobstructed. The Cyberleader shows no sign of registering anyone other than the Master on his visual receptors.
When Susan leaves the (First) Doctor to look around, she takes about 6 steps, and tells the Doctor to come and see what she's found. She has found the TARDIS, which is sitting in the middle of an open plain, with no hills, trees boulders, or anything else in the way. It is inconceivable that the Doctor could not see the TARDIS from an additional 6 steps further away, or that Susan actually had to take those 6 steps to see it herself, since there is absolutely nothing it could have been hiding behind.
The Castellan is shot dead by the guards and his body is lying on the floor. As a Time Lord, he should have begun to regenerate.
The Third Doctor uses a zip-wire to 'rappel' (slide) himself and Sarah to the tower. However this would not have worked as there is a noticeable sag in the middle of the line so they would have gotten stuck half way across.

Plot holes

Borusa claims he needed the Doctor(s) to pave the way through the tomb of Rassilon so that he could then acquire the Ring of Rassilon to achieve immortality. However, he and the Fifth Doctor simply transmat themselves directly into the inner chamber of the tomb and so the Doctors' involvement seems to be completely pointless to Borusa's scheme.
When the Master crosses the deadly chessboard floor inside the Tower, he tells the Doctor "it's as easy as Pi", referring to the mathematical formula. Firstly, there is no way for him to know that, and secondly, it is obviously nonsense and he, then the Doctor and Tegan just move across random squares.
The little figurines of the five Doctors and his various companions that President Borusa places onto the lit pentagonal panel serve absolutely no purpose and are meaningless to achieving his plan.

Character error

When the Second Doctor and the Brigadier meet Jamie and Zoe, he realises they are not real as the Time Lords had wiped their memories of the Doctor and the time they spent with him. However, at the end of the War Games, The Time Lords allowed Jamie and Zoe to remember their first adventure with the Second Doctor.
Lord President Borusa uses the Coronet of Rassilon to force all the companions to remain still, ordering "Be silent, all of you! Do not move or speak until I give you leave." All do so, staring blindly to where he stood when he gave the order, except for the Brigadier, who turns his head tracking Borusa's and the Fifth Doctor's exit.
After Chancellor Flavia informs The Doctor that he is now President, he tells her to return to Gallifrey and summon the High Council. But they are already on Gallifrey since that is where The Dark Tower is located. [Which they are in]
It seems odd that doctors two, three and five are so reverential towards the first as technically he is the youngest of them all. He may be the original but surely he should be more respectful to his older selves, he treats the fifth doctor like a small child.
