Continuity

The fold, in the photo of the Eye's daughter, moves around when we see it in close-ups.
After the Eye beats up Gary the amount of blood on the Eye's hand changes. From some when he reaches for his gun to point it at Gary, to none when he rolls Joanna over, to drenched when he's talking to Joanna.
When the camera swings behind Stephen as he watches Joanna leave the hospital, there's a cigarette dangling form his lips, but when the camera changes immediately to a front shot, there is no cigarette.

Factual errors

A scene shows an electrified train leaving Pittsburgh. The PA railroad only electrified as far west as Harrisburg.
A Chicago policeman says he's with the Federal Police. The United States does not have a federal police force.
The Chicago Police cars are blue and white, not red and white, as depicted in the movie.

Revealing mistakes

The first scene in movie is supposed to be in DC but all the cars have province of Quebec plates revealing its Montréal location.
A little past the halfway mark, when the Eye spots the newspaper headline and subheading about Joanna/Charlotte Vincent and Alex Leonard, nothing in the columns below is about them.

Miscellaneous

The end credits state that no animals in the film were "mistreaded."
