Uri's hair color and style changes constantly throughout the film with no explanation.
After Paul mauls the man outside the bar, he briefly fights a random pedestrian who rushes up to stop him from off screen. The person he is fighting is clearly Yuri without his jacket on and wearing a cap. However, Yuri appears in the bar after this scene with no signs of having ever left the establishment and missing the hat and still wearing his jacket.
When the "Girl in Jeep" runs away from the werewolf, her dress is already muddy just before she reaches the mud puddle.
When Yuri and Natalie are playing pool, she pockets three solids before inexplicably turning to stripes.
The close-ups of the werewolves look absolutely nothing like the wider shots.

Factual errors

The full moon is shown occurring for far too long a period than is possible in nature.
The skull of the werewolf has fossilized ears. Supernatural or not, a wolf's ears, or any other type of ears for that matter, are made out of skin and cartilidge, which dries up and decays away instead of being fossilized.
An Amtrak commuter train is seen in Flagstaff. That city is not served by commuter rail.

Revealing mistakes

During the driving scene where Yuri follows the security guard/werewolf, both drivers pass the same gas station twice. Also, Yuri turns the steering wheel like he is making sharp turns when he is driving straight.
When Yuri beats Paul with the werewolf skull, he clearly misses by several inches multiple times.
The feet on the werewolf skeleton are one solid piece, visible when it's first on the table. Actual foot bones would be separate, unless there was still skin stuck to them.
Director Tony Zarindast used stock footage for the car wreck that he had purchased before the shoot. In an attempt to make the footage match with the car used for the shoot he had the car painted the matching color of the stock footage car. Still it's obvious that the car is a Ford Taurus and the car in the wreck is a Plymouth Acclaim.
During the fight near the beginning, punches are clearly missing by several inches.

Audio/visual unsynchronised

When Paul is chasing after the girl in the jeep, after she "falls" in the mud, the girl screams. However, reading her lips clearly shows that she is saying, "Oh my god!"

Crew or equipment visible

When Paul goes on his rampage out of the bar, he knocks out a man as soon as he exits. When the camera comes in for a close shot of the man, you can clearly see the shadow of the camera operator on the man's shirt and also the camera operator kicking the man.
