Catherine's hair changes throughout the movie, appearing to be a cheap wig in some scenes. When speaking to the Reverend, her hair looks fake and is all one color. When battling Lilith at the end, you can see dark roots in her hair.
When the Reverend finds Lilith disguised as Catherine in the basement, after she reveals herself, she moves the torture device up. It shows a shot from the back and you can see Catherine's clothing, even though Lilith turned back to her original form.
The blood on the key disappears in some shots and reappears later.
When Rafe enters the Gaines crypt in the cemetery, a stick is visible pushing the "flying cat" off of its shelf.
Obvious doll when McCutcheon is run down by a car.
When Guttman is fighting Caleb, the blu-ray or DVD allow viewers to clearly see Vincent and McCutcheon walking in the background since they just arrived at the power plant and are seen moments later with Catherine. Originally it was unclear on TV and VHS copies and it appeared to be a crew member. Now viewers can see it's not a goof.
