When Montgomery rips pages out of a book, his shirt is blue. When Azazello takes the gun from his hunting vest, his shirt is white.
Before Montgomery injects Douglas' I.V., Montgomery places the syringe cover between his teeth. After the injection, Montgomery grabs the syringe cover from the bedside table.
When Douglas is first walking through the creature settlement, his shirt sleeves are rolled up. In the next shot from the same scene, his sleeves have gone down
When Edward Douglas is rescued from the life raft, the shadows in the raft seem to indicate a different time than would be indicated by the position of the sun behind the rescue boat. If he can see the rescue boat, the sun is in two completely different positions in the sky.
After witnessing the birth, Aissa guides Douglas to the beach. As they run through the forest, the light coming through the trees casts long shadows for the characters. When they emerge onto the beach, they have no shadows at all.

Factual errors

The ship's name is Ombak Penari, which in Malay and Bhasa Indonesian translates as Wave Dancer; ombak means wave, penari means dancer. The grammatically correct name would be Penari Ombak.

Incorrectly regarded as goofs

Dr. Moreau did not invent velcro; George de Mestral did. However, it is clear that Montgomery is lying to Douglas when he says that. He lies because he doesn't want Douglas to know what kind of scientific research Dr. Moreau is actually doing in the island.

Crew or equipment visible

When Azazello catches Douglas and Aissa in the underground bunker at the end, David Thewlis' stand-in, dressed in identical clothes, is very clearly in his position in the wide shot.
The shadow of the cameraman is visible as the camera changes views of Edward while he is thinking of how to kill the other animals.

Plot holes

Surely after learning that a beast person had removed his implant, Montgomery would have informed Moreau, in which case security measures at the compound would have been tightened. At the very least, Moreau would have known not to rely solely on his remote for protection.

When Montgomery is putting the rabbits in the cage.
