Krazy K shoots Deke twice, but Deke ends up with three bullet wounds.
Duke partially closes the door to check for the little devil doll, but in the next shot it's in its original position.
When Morehouse chases Strom and Newton to their cruiser, he's at the window, leering inside. But as soon as they drive away, Morehouse is about 10 feet away from the cruiser.
When the teacher Richard introduces Walter to the class, his face is fine. After the schoolyard fight with Ty, the school nurse tells Richard that the black eye Walter has is a couple of days old.
In the tale of the crooked cops: The name of the murdered man who seeks revenge is listed in the police database as "Martin Ezekell Moorehouse", but his tombstone reads "Martin Ezekiel Moorehouse".

Factual errors

Tom Everett is not in the movie despite being listed prominently in the opening credits.

Incorrectly regarded as goofs

When Officer Newton becomes part of the mural, his name tag says "Hauser." This is because the officer's full name is Newton Hauser.

Revealing mistakes

When the cop pees on Martin's grave, the mustard bottle used to simulate urination is visible.
Obvious blood packs under T-shirt of fellow whom Krazy K shoots.

Miscellaneous

In the opening scene when Mr. Simms freaks the boys out when raising the blinds, he's wearing glasses. In his next close up, he isn't. In several trailers and TV spots for the film, a deleted shot shows him quickly removing his glasses as the boys react to his sudden appearance.

Audio/visual unsynchronised

We hear Strom unzip his fly twice before he urinates on Martin's grave.

Plot holes

In the first story, the orderly explains that Clarence was a cop-killer and he took out three in one night. But Newton was turned into graffiti and no one would realize that he was killed without a body.

Character error

When Clarence is on the hood of the police cruiser, he says "I actually believed that Strom and Billy were gonna take Morehouse to the hospital." But he's talking to Strom at the time.
