In the flooding scene in the slave quarters the Candyman's hook falls off into the water during one of the shots. In the next shot of the Candyman swimming, the hook has reappeared.
When Daniel's hand is cut off, they show that it was sawed off right around the wrist, and the hand alone falls to the ground. In subsequent shots in the same scene, his whole forearm is missing.

Revealing mistakes

As Annie's brother falls down the steps, he is obviously replaced by a stuntman with long hair.
When the Candyman throws the detective through the window, the glass that was whole before is not when the detective starts to go through the window. It is pre-shattered like safety glass. The first image you see of the officer going through the window, after Ethan is shown cowering in the corner, his elbow has already impacted the window. (This can be seen when watching it frame by frame.)

Character error

If Annie and Ethan lived their entire lives in New Orleans, they should speak with a Louisiana accent, but neither of them do.
