In the hospital scene, where Doug is menacing the chauffeur for information, he draws his S&W double-action revolver. Even after firing it, however, he is shown to cock the hammer repeatedly, as though it were a single-action gun.
When Chesnik is grilling Earl about the location of Mrs. Carlisle he shoots one of his toes. He begins counting down to shoot the next and cocks his revolver but does not shoot. When he asks Earl "An abandoned farmhouse? I don't think so!", flashing the gun in Earl's face. It is uncocked.
When rescuing Mrs. Carlisle from being buried, the breathing tube her kidnappers installed would've been far too long. She would've suffocated due to what's known as the dead air effect. A snorkel can only be a maximum of 16" before rebreathing more CO2 than fresh air becomes an issue. Her breathing pipe was well over 3'.
The "Ohio" state route signs in the movie are wrong, as the real ones have the outline of the state. The signs are Maryland signs (where the movie was filmed) with the word "Ohio" covering the word "Maryland".
The plot includes two interactions with the local sheriff's department. Uniforms and vehicle markings for all 88 counties of the 7th most populated state of the US are standardized and dictated by Ohio law. The uniforms and cruisers depicted are not remotely close.
The story takes place in Ohio but every car throughout the movie has Maryland license plates where the actual filming occurred.
When reporting the last know location of Mrs. Carlisle's Lincoln, Agent Chesnic reports her vehicle heading East at the Chester Exit off Interstate 71. However, Chester, Ohio is located in Rural Southeastern Ohio; nowhere near an interstate.
At the gas station where Doug arrives & is picked up from after the kidnapping, a pair of (clearly faked) highway signs depict Ohio SR 137 as running EAST and WEST; the actual (extremely short) road is oriented north-south.
When Doug is delivery the breakfast tray to Tess in bed, the boom mic is briefly visible.
Frederick sarcastically expresses surprise to Doug that he isn't standing outside The Russian Tea Room as part of Jacqueline Kennedy Onassis's Secret Service detail. Kennedy forfeited her Secret Service privileges when she remarried.
