Continuity

The satellite on top of the RV disappears once the TV has been turned off.
At the start an extra appears from nowhere to the left of the red car when they are leaving and the shot changes.
After having hit Teddy, Ray leans over to start the RV without having shut it off in the first place.
When Fallon knocks Frank into the stack of Old Style beer cases, it's plain to see all the boxes have nothing in them.
When they climb out of the sewer, their pants are not soaking wet despite them spending several minutes standing in knee-deep water. Also, their shoes should have been waterlogged to the point where they would leave wet footprints on the sidewalk, but they don't.

Revealing mistakes

When Frank carefully steps over the fallen body of Fallon, Fallon's eyes slightly follow the camera even though he's supposed to be dead.

Miscellaneous

During the opening shot that pans from Mike's car to Frank's house, you can see trees that don't belong in the supposed location of Evanston, Illionois, including a very tall palm tree in a neighbor's yard. The scene was likely filmed in suburban Los Angeles.

Audio/visual unsynchronised

Before Fallon guns down Teddy, one of the questions he asks him is, "All you had to do was keep the merchandise moving Teddy, but what was the problem?" We see Fallon's face while he's talking but the way his mouth is moving doesn't match those lines that are heard.
The punch sounds during the boxing match on television were unrealistic for a boxing match and actually louder than the rest of the television.

Crew or equipment visible

When Frank finds the dead cop, there is a crew person and a cameraman reflected in the window.
