At the end of the film, Billy can be seen running over Montoya's shoulder, yet over Giles' shoulder, he's not there.
At the beginning when the woman is pushed out of the window, she flips while falling so that she would land on her back with her head toward the building. When the news camera zooms in on her body, she is lying on her stomach with her head away from the building.
A terrorist shoots the phone booths as a student is calling 911. At the end of the film, when Cali is dragging Billy to the office, the phone booths are undamaged.
In the opening run, Joey is first seen where white shorts like the other boys with bare hands. but when he's standing watch and after, he's in black shorts with goalies gloves on.
At the end of the film, when Billy is running, Hummers are not present, then parked, then just arriving.

Factual errors

Getting hit with a burst from a 30mm chain gun results in man's body getting completely torn to pieces - not receiving only small bullet holes as from conventional small arms.
In the closing scenes, Billy and Dean Parker are on a bench in the hall while special forces personnel mill about with disarmed bomb parts and wires. No special forces or bomb squad in the world would risk disarming the bomb while civilians were still in the building. Particularly as the next shot establishes all the students leaving the building, indicating all 91 other boys were still in the building too (or at least in the basement).
The U.S. government would never intervene in any hostage crisis on American soil it's in the article of the constitution that American forces are never to be deployed on American soil unless invaded by a foreign military local police swat and the FBI tactical terrorist division could have easily handled the situation at the school not the U.S. Army..

Incorrectly regarded as goofs

The bathroom door appears to open both ways even though the jamb has a stop; in fact, it's a double door.

Revealing mistakes

After the terrorists drop the judge from the helicopter at the beginning, you can clearly see he's wearing a black parachute.
The wire used to pull the airplane straight upward is visible in the final battle scene.
There's a slight delay between when the two terrorists on the rooftop get hit by the snipers' shots and when they actually jerk and fall from the hits.
A mat that protects Sean Astin's arm is visible when Billy is lying down on the ground.
The gas flame used to simulate muzzle flash can be seen coming out of the 30mm chain gun on the Apache attack helicopters.

Miscellaneous

At 1:40:20 while the soldiers are assaulting the school, you can see the final soldier leaving the helicopter wipe out.
Billy barely makes it back to the school in time for the hourly count, using his wetness (from falling into muddy water) as a cover for supposedly having been in the shower and not hearing the bell ring. Cali inspects him and confirms he's wet, seemingly convinced, but standing right next to Billy, Cali would have noticed he smelled like muddy water, and certainly not like someone who had just cleaned himself in the shower.

Audio/visual unsynchronised

When Cali fires his pistol at the Soldiers trying to get into the headmaster's office while holding onto Billy, there are more shots heard than are actually fired.
When the Blackhawk helicopter comes into a hover over the quad, muzzle flashes from the guns of the terrorists on the steps can be seen, but no shots are heard.
When Cali shoots the teacher, there's no muzzle flash or smoke to indicate the weapon was fired, yet the sound is heard.

Crew or equipment visible

Shadow of the camera is visible when moving around the furniture in the garden.
At 1:16:02 in the terrorist Carlos flinches before Joey even hits him.

Plot holes

The Feds at least suspect Cali's plan to kidnap the Phil Donoghue, since they remove him from the school. The Feds could have protected the school by either announcing that Phil was removed, or by leaving a law-enforcement/military presence to defend it. Bizarrely, the Feds do neither.

Character error

Luis Cali is told that there is a power failure affecting the whole county (to keep him from seeing the news about his father on TV). However, two scenes later, the lights are on over the sinks and on the ceiling in the bathroom adjoining the school dining room.
When they take Enrique Cali out of his cell the Guards Shackle his hands and feet. When they put the leg irons on him you can see he has shoe laces on. No jail or prison would allow inmates to retain or have shoe laces as they can be used for suicide or as weapons (strangling someone).
During the final battle when the 2 black hawks are on the round and about to lift off you see a soldier fall over on the left of the screen, possibly from the downdraft of the helicopter
When the terrorists first enter the school and shoot the ceiling, the man with glasses fumbles putting the clip back in the gun. A well trained Colombian terrorist would have no difficulty in reloading a weapon he is so comfortable using.
