In the opening scene, two ninjas are shot by a Delta Force soldier using his pistol, yet other ninjas are later shown able to dodge pistol bullets easily by pivoting, while still others are shown to magically avoid machine gun fire without even having to move at all.
At least twelve ninjas directly face Sean in the forest. He kills three of them with his bow and arrow, and then simply jogs away, with none of the other ninjas tracking him anymore, suddenly. Instead, they are all slowly heading in another direction, inexplicably.
During the final rescue, one of the ninjas is thrown through a burning fire and made to catch fire, with him screaming in pain and writhing around. A moment later, his screams are completely cut off when the camera shot changes, as if the shot has cut to a distanced area, but it only cuts away a few feet, and the screams should be fluid and still heard.
The evil super ninja goes through three costume changes:

The first is briefly visible during Delta Force's escape, when Mulgrew is standing on a tall cliff as he watches the action play out from afar through binoculars. The super ninja is seen standing next to him wearing an eye patch on his left eye, a large white/silver colored chest guard, a silver belt with a black X shape on it, and matching silver hand guards. He also wears no hood, and his large, frizzy white hair is showing.

The second costume is seen during the evil ninja training scene. The fabric of it is a mixture of flat black and metallic black colors. He now wears a metallic hood that covers up his hair, has a flat padded waist wrap, and there is one red stripe outlining the inside of his metallic vest. The fabric of his pants and boots are flat black while his forearms are metallic. Also, his eye patch covers his right eye, and there's a small portion of his white hair sticking out from underneath his hood that hangs down just over his left eye.

The third costume is seen during the execution/final battle with Joe, and is of a more traditional karate type gi. The fabric is now all flat black in color with red stripes lining the inside and outside of his (larger) vest. There's also a large diamond shaped patch sewn on to his hood, with a long tassel hanging down the back side of it. None of his hair shows as he now has a red bandanna covering the bottom of his forehead. His belt is a traditional karate type that wraps around his waist and knots at the front, while, finally, his eye patch is still covering his right eye.

Revealing mistakes

When the ninja jumps on the front of the car, Joe punches his hand through the windshield, but if you watch carefully, you'll see that the glass breaks seconds before his fist makes contact. It evens falls inward instead of going outward.
When the Delta Force commando throws the grenade which explodes by the two ninjas, the explosion can be seen to be emanating from a black box generating flames, with the camera placement almost completely concealing it from view.
Joe slashes the throat of a ninja and throws the same knife into another ninja's side, but upon careful examination, it is clear there is nothing in his hand.
The explosion from Joe's grenade does not originate where the grenade lands, but rather a few feet away, on the outside of the crates.
The ninja who gets impaled by the spear in the obstacle course clearly is not touched by it, but rather grabs it and jolts his body toward it to aid in the illusion of being stabbed.

Miscellaneous

After an arrow almost hits Sean out of nowhere in the forest, he prepares to do battle with whoever shot it, which turns out to be an entire ninja army. Inexplicably, he decides to slowly prepare his weapons and meditate, in clear and present danger, while they the ninjas hold off on attacking him for no good reason.
A red ninja in the back of the group can be see losing synchronization with the rest of the group during the ninja army exercise.
Sarah insists on joining the CIA mission with Braxton and Sean, insisting Mulgrew and his men will be pursuing her, yet she has no reason to believe this after O'Reilly having left the premises without issue.
Braxton decides to fire a rocket at Ali's helicopter, despite not knowing who is on board, risking killing innocents, and in fact killing one: the pilot, who is not established to be a bad guy in any way.
Throughout the film, ninjas are constantly knocked out for incurring minor injuries or trips.

Crew or equipment visible

In the opening scene, right after the commando shoots the two ninjas with the bows and arrows, the shadow of the camera is clearly visible on the ground, in the left-hand corner of the screen, right behind the running commando.
In the opening scene, as the Delta Force commandos hear the sound of one of their comrades screaming, a crew member's lap (wearing jeans) is perfectly visible in the bottom left-hand corner of the screen.
When Mulgrew shoots the stool out from underneath the heavyset man to hang him, he does indeed hang in the air, with a noose around his next. The noose, however, is clearly a separate rope from the one hanging from the ceiling, while the rope hanging from the ceiling and attaching to the man is clearly going into his clothing, down the back, attached to a girdle of some kind, ensuring to suspend the man, while the rope around his neck gives the illusion of being one and the same rope, asphyxiating him.

Character error

In the first scene when Michael Dudikoff appears, he is teaching a group of children in a church, as he starts the class, he writes "environment" in the board and reads it out loud, but he reads "en-vi-OR-ment" when it should be "en-vi-ROn-ment".
