When Dr Allenby is trying on her "murder outfit" at her house prior to going to the party, she is shown putting on a black silk undergarment (covering her chest). She then puts on a black dress. This dress has deep "V" neck which goes down to her stomach showing her skin - the black undergarment has disappeared.
After David gets shot and falls to the floor, he falls on his right arm and it is not visible but in successive shots his right arm becomes visible and then it becomes fully extended.

Factual errors

At the beginning of the tuba playing scene, the man who introduces Columbo is playing a brass instrument which he refers to as a tuba. This is incorrect; he is actually playing a Sousaphone. (The instrument Columbo plays is indeed a tuba.)
Columbo states that the gun was purchased from a dealer on the day of the murder. California has long had a required a 10-day waiting period on new handgun purchases. Unless the dealer was also dishonest, it would be impossible to purchase a handgun and leave the store with it on the same day except for limited law enforcement exceptions.

Revealing mistakes

Not surprisingly, the fingerings Columbo uses on his tuba have no relation to the notes he is heard "playing."
When Columbo is "playing" his tuba while marching with the kids and the Sousaphone player, the instrument heard playing the melody is not a tuba but a baritone horn.
When Dr Allenby returns to her office and sees her lover David Kincaid in bed with Cindy Galt, Galt is wearing an obvious flesh-colored bodysuit.
When Dr. Allenby returns from Chicago while driving with David, a bicyclist is seen on her passenger side. He pedals faster than the car while David is driving down an empty street.
After Columbo talks to the bartender, he tells Joan that Lisa had turned down another man before picking up David, yet the bartender had not told him that. While it might be surmisable that the entire conversation was not shown, the bartender was very descriptive, detailed and sequential on all of Lisa's movements, but he failed to mention the man she turned down. Columbo could not have known that detail from the bartender. And there's no indication that he had talked to anyone else from the bar. Columbo is not the type to just fill in facts he doesn't know.

Miscellaneous

When Dr. Joan Allenby (Lisa) lures David Kincaid back to the therapy bedroom, right before she shoots him she slams her fist into a picture frame mirror that's freestanding on a table, shattering it. Clearly the mirror would have flown off the table.

Crew or equipment visible

Equipment can be seen moving back and forth in a reflection from the desk of Dr. Joan Allenby, when she is in her office with David Kincaid just after the start.

Boom mic visible

Around 59 minutes in, the reflection of the boom pole and mic is seen in a small round mirror on a dresser behind Columbo.
