When Ed picks up the unconscious Billy after the accident, the kid's arms are around his neck as he walks away. When it cuts to the front view, the arms are down by Billy's sides.
In the scene where Ed Harley is attacked by a dog while confronting Pumpkinhead, the dog changes which arm he is biting without notice.
Maggie is shown wrapped in a blanket, but in the next shot of her few seconds later she is not wrapped in it like before and no movement on her part was heard.

Factual errors

Billy has been dead for a few hours by the time Haggis conjures Pumpkinhead. She would not have been able to draw blood from the dead boy's hand because once the heart stops beating, blood pools at the lowest point of the body.

Incorrectly regarded as goofs

When Ed Harley enters the cemetery, he is startled by a vulture perched there. Vultures are not nocturnal birds, so it should have been sleeping in a nest in a high location.
Since Ed seems to be affected by what happens to Pumpkinhead, and vice-versa, he should have fallen to the ground when he shot it. *That's not the way it works-the monster can only be injured or killed if its host-Ed Harley-is injured first. He also sees through the monster's eyes; it doesn't appear to see through his.*

Revealing mistakes

In the scene where Pumpkinhead is entering the burned out church, he takes several steps where you can clearly see that he's wearing Nike shoes. (FULLSCREEN ONLY).
When Steve is lying dead on the cabin floor just before he is covered up by his brother Joel you can see his eyes moving.
When Haggis the witch cuts Ed Harley's hand to draw blood for the resurrection of Pumpkinhead you can clearly see the blood running from behind the knife and onto Ed's hand, and not out of Ed's hand itself.
When the old woman is resurrecting Pumpkinhead, she cuts Billy's wrist or hand and blood trickles out. As he has been dead for several hours, this would be impossible.
Wires visible several times when Pumpkinhead picks up Bunt by his leg.

Miscellaneous

When Pumpkinhead picks up Bunt by the leg, the tops of Tom Woodruff Jr.'s sneakers are visible at the bottom of the frame. Woodruff notes this during the audio commentary for the special edition release.

Crew or equipment visible

After Mr. Wallace (George 'Buck' Flower) tells Ed Harley (Lance Henriksen) to "go bury your boy," one of the Wallace kids stops Harley while he is driving away. As Harley and the Wallace kid are talking, the top of the camera operator's head is visibly reflected in the truck window. It becomes obvious when the operator lifts up their head to look at the actors, before looking back down into the camera's viewfinder.
