- In New York City, Dr. Morris Ackerman returns from a gourmet delicatessen to the apartment he shares with his wife, Martha, with animal brains. Martha gushes that "Elmer," a talking, one-foot long parasite with blue eyes, will love them. She enters the bathroom to feed Elmer and finds the parasite missing. Morris and Martha frantically search for him. After tearing the place up, they fall to the floor and foam at the mouth.
Meanwhile, the Ackermans' neighbor, Brian, is in bed under the covers. Barbara, Brian's girl friend, enters to awaken him for a concert. However, Brain has a headache, and suggests she attend with his brother and roommate, Mike. Barbara and Mike leave, and Brian returns to bed. When he wakes up again, he notices blood dripping from the back of his neck. He starts to hallucinate blue water flooding his bedroom, and a bright, light-filled eye staring down at him. He stares back entranced.
In the bathroom, Brian sees a bloody wound on his neck. Suddenly, Elmer slithers up Brian's back, and says he will keep giving the euphoric "light" if Brian will take him outside. Brian agrees, and attaches Elmer to the back of his neck. Elmer opens his month and punctures Brian's neck with a needle-like tongue, depositing a small amount of blue liquid into Brian's brain.
Outside, Brian runs around a car junkyard, seeing an array of lights and colors. When a security guard searches Brian for weapons, Elmer latches onto the guard and burrows into his head. Still under the influence, Brain watches Elmer eat the security guard's brain, killing him. Afterward, Elmer instructs Brian to take him back to the apartment.
A few days later, Barbara talks to Mike about Brian's erratic behavior. She knocks on the locked bathroom door as Brian takes a bath with Elmer to remind him about their dinner date. Later at the restaurant, Brian tells Barbara about his new senses and the glowing lights. Attempting to tell Barbara about Elmer, the parasite bites Brian while hiding under his clothes. He leaves the restaurant and stumbles into an alley, begging Elmer for more juice. As the serum drugs him, Brian walks into an underground rock club called Hell. A blonde woman approaches, leads him outside, and kisses him. Afterwards, Elmer attaches himself to the woman, and kills her.
When Brian regains his faculties, he leaves the club, confused by the blood on him. Brian is accosted by Morris, who demands to know where Elmer is. Morris reveals the creature's name is the Old English word "Aylmer," meaning all inspiring and famous, and has been alive since the Crusades. Brian runs from Morris and returns to his apartment. He packs his clothes and runs away with Elmer. Brian rents a room at a hotel. He places Elmer in the sink and asks about Morris. Elmer tells him Morris and Martha fed him animal brains instead of human ones to keep him weak, and they were addicted to the blue juice injections. Brian asks about the blood he found the night before.
Elmer reveals he ate the woman's brain, as well as the security guard's. Horrified, Brain insists Elmer stop killing people. However, Elmer states that if he does not get brains to eat, Brian no longer gets juice. Hours later, Brian foams at the mouth as he detoxes from the juice. Unable to resist, Brian begs for more. Elmer agrees, but demands Brian find a brain first. Brian enters the hotel's communal bathroom with Elmer under his towel. Elmer kills a man in a toilet stall. Afterward, Brian leaves the hotel and returns to his apartment.
Later, Mike and Barbara arrive after looking for Brian. Mike confesses his feelings for Barbara, and kisses her. Brian, overhearing them make love, attaches Elmer to his neck, and is injected with blue juice. He visualizes making love to Barbara himself and eating her brain. Afterward, Brian walks out of his bedroom and tells Barbara and Mike that he needs to feed Elmer, and, because he cares about them, they need to be gone when he returns. He goes to the subway station, followed by Barbara. On the train, she confronts Brian about his behavior. When she kisses him, Elmer attacks her through Brian's month, ingesting her brain and killing her. Later, Morris and Martha confront Brian outside the apartment.
As Morris aims a gun at Brian, Martha pulls Elmer off of him. However, Elmer attaches himself to Martha's head and eats her brain. While Morris attempts to help her, Brian grabs the gun, and Elmer burrows into Morris' head. When Elmer is done, Brian reattaches the parasite to his neck. As Elmer injects Brain with blue juice, Morris grabs Elmer, causing a large injection of juice into Brian's brain. Before he dies, Morris strangles Elmer, killing the parasite.
Later, Mike opens the apartment door to Brian who is screaming with a pulsating bump on his head. Brian runs to his room and aims the gun at his own head. The other tenants approach two police officers about the commotion. At the sound of a gunshot, they look up to Brian's window and see flashing lights. When Mike lets the officers into Brian's room, the top of his skull is missing and his brain emits a white light.
