When in the plane going to the training camp the plane has windows in external shots and no windows in internal shots
The length of the chain securing the briefcase to the Courier's wrist becomes longer once the Courier is put into the closet and the door is shut.
When the Russian soldiers are listening to the music around the campfire, once the flares go off the music stops, yet no one touched the audio device.
The missile is seen climbing through heavy clouds but later, when we see it from the ground, the sky is clear.
During the sky shot of the missile launch, it's obvious that it's a mini model set.

Factual errors

The ambulance, in which Emmitt and Austin escape, has improper markings for the Middle East. It carries Red Crosses, when it should have Red Crescents.
End Credits acknowledge Eddie Bauer sportswear for their contributions. They are in "Redmond, Washington" but the end credits say "Redman, Washington".
The gold-bordered, blue ribbon which Generals Sline and Miegs wear on their right side is the Presidential Unit Citation. While it is correct for U.S. Army personnel, such as General Miegs, to wear the Presidential Unit Citation above the name tag, U.S. Air Force personnel, like General Sline, actually wear the ribbon on their left side, along with all other ribbons and devices.
At the end of the movie the recalled ICBM explodes at 3000 Zulu time. Zulu time is the same as Universal Coordinated Time (UTC) and operates on a 24-hour clock. In a 24-hour clock the day runs from midnight to midnight and is divided into 24 hours, indicated by the hours passed since midnight, from 0 to 23 (e.g. 1600 hours is 4:00 P.M). 3000 hours is an impossible time in a 24-hour day.
Fitz-Hume and Millbarge are dropped out of a C-119 Flying Boxcar into Pakistan, but this aircraft was retired by the US military in 1974.

Incorrectly regarded as goofs

Emmett does not appear to receive any test papers before the monitor starts the exam. The monitor gives two sets to Austin, who gives one to Emmett.

Revealing mistakes

During the ambulance chase scene, an Afghani jumps from a horse to the back of the ambulance. An Afghani rolls from the back of the ambulance to give the audience the impression that he didn't make the jump. But if you look closely, you can see the first Afghani's headdress.
The scene after the checkpoint cabin blows up from grenades. Dan grabs a horse and they ride off together with the horse that had a white spot on its forehead and is a light brown color. The next scene they are walking with a different horse that has a long white strip on its face and is darker.
When they are watching the camera footage of the "heart attack" in the examination, there is a shot taken right in front of Fitz-Hume, to do this there would have to have been a camera on the floor in front of him, which there wasn't.
The text in Cyrillic above the launcher monitor is a nonsense. In fact it's just a chaotic mix of Russian letters. Text on the screen is, however, in correct Russian, but related to ship navigation and not to missile control.
At the beginning, the secret courier holding the satellite photos walks up to a building, where the exterior windows do not match the interior windows. After he is inside, we can see the windows are spaced farther apart.

Anachronisms

After Austin and Emmett are picked up in the Jeep by the two Russian agents and stop in the desert, Austin discovers that they are not their legitimate contacts by the digital LCD wristwatch one of the agents is wearing. Austin tells Emmett that the agent is wearing "a Russian copy of a 1969 Timex digital". Timex was not producing an LCD digital watch in 1969. In fact, nobody was. Digital LCD watches were not introduced until 1972, and Timex did not produce one for many years after that.

Audio/visual unsynchronised

When Fitz-Hume throws the live grenade back into its box and the Russians notice, two seconds before they scream you can hear a faint leading echo of that scream. This is due to use of spooled analog magnetic tape when magnetic state on one layer of tape in the spool influences the state of another. It is most noticeable after long silences like this scene. This flaw survives uncorrected even to HD versions shown on cable.

Plot holes

During the "negotiation" scene all Russians are shown as awarded two times Golden Star of the Hero of the Soviet Union which is a highest Soviet award. Assuming they all where awarded by one for "saving the world" it's unlikely to believe they all committed another deed of the same scale before.
