When Golgo 13 fires his revolver at Gold and Silver as they leap at him, Silver's prosthetic left leg is not drawn correctly, and looks like his normal right leg.
When Golgo 13 fires at the Union Company's windows, the company's logo changes from being written in a serif font with gold paint to being written sanserif with white paint. The latter logo also loses the former's period at the end of the company name.
When the Computer 1 Operator researches the Union Company's floor within the USA Building, the onscreen text that reflects onto his face reads "corporation" instead of "company".
When Cindy falls to the ground after being shot by Golgo 13, her neck wound is not visible.
When Cindy confronts Golgo 13 on horseback, she is armed with a M11 sub-machine gun (the same kind used by her bodyguards), despite not procuring one before she began riding to meet him.

Factual errors

The Ford dealership that Rita runs in San Francisco displays models (including the modified Ford Laser hatchback Golgo 13 uses in his getaway) that were produced for the European market only. The only exception in this case is Rita's Ford Thunderbird.

Audio/visual unsynchronised

Throughout the Japanese version, many of Snake's vocalizations (especially his cackling laughs) are completely out of sync with his lip movements. This is less common in the English dub, which puts a greater effort into ensuring that his voice is synchronized.
When Cindy tells Golgo 13 that he is the first Asian she has met in the Japanese version, the last part of her line is spoken without her moving her lips. This does not occur in the English dub, which (in addition to altering the line so that she instead says that he is the first Asian she has slept with) has the last part of her line being spoken off-screen.
When Golgo 13 tells Pablo of their next rendezvous, he moves his lips for several moments before his dialogue is actually heard. The English dub attempts to rectify this by additional dialogue to fit the mouth flaps, but introduces another error by having Golgo 13 speak the last part of his line *without* moving his lips.
When Golgo 13 is assembling his M16 to shoot Robert Dawson, no sound is heard when he attaches the stock to the firing mechanism. The English dub corrects this by replaying a metallic clicking sound heard earlier when the rifle was kept off-screen.

Plot holes

The three special agents sent into the church (one with a hooked hand, one with two ice picks and one with a knife) to kill Golgo are explained to have been killed in the firefight that ensued when Bragan's team unloaded on the church. Later, when Bragan tests Snake's ability, the same three special agents appear to fight him.
How does Golgo 13 know that Don Hughes will be standing directly in front of the Hughes Penthouse's window when he fires, especially given that the USA Building which he fires through obstructs his view of the penthouse?
When The Clockmaker is gathering intel on Dr. Z for Duke at the beginning of the movie, he says that nobody really knows who Dr. Z is, but he gathered footage of the man's family anyway. If nobody knew who Dr. Z was, then there was no way to know who his family was, either.
