When Barnes is picking up the Ciello family at their home, Barnes puts his hat - a brown fedora - on Ciello's young child's head. In the next shot, when Barnes takes his hat back from the child, it is not the fedora he had just put on the child but instead is a dark hunting hat with a long baseball cap like bill and no brim.
Early in his Chase Commission assignment, Det. Ciello shaves his chest hair to prevent the tape holding the wire to his chest from pulling out his chest hair. However, when he arrests bail bondsman DeBennedeto, Ciello's chest is shown to be unshaven.
Gino Mascone's wife's name is listed as Ann in the credits, but she is repeatedly referred to and addressed as Rose in the film.
Near the start of the film when Dan is pushing his brother Ronnie around, there's a large crack on the wall (probably from a previous shot). After he pushes Ronnie near the wall, another large crack appears but Ronnie is never shown hitting the wall.
When Danny is arguing with his brother Ronnie, Ronnie leans out an open window to yell at Danny's partners. But when it cuts back to inside the room, one can clearly see the window is closed.

Factual errors

Assistant U.S. Attorneys Cappalino and Paige discuss emptying the entire SIU Narcotics unit at one time through "normal rotation". However, Det. Ciello had been there for 11 years and Det. Mayo had been there for 9 years, so clearly there's no such thing as "normal rotation". Moreover, an entire Narcotics unit would not be routinely emptied all at once, destroying institutional memory; members would come and go individually, through attritional reassignments and retirements.
Chase Commission special prosecutor Richard Cappalino's credentials show his job title to be Special Assistant U.S. Attorney, yet he introduces himself to Detective Ciello as "District Attorney Cappalino" - which a real Assistant U.S. Attorney would never do.

Revealing mistakes

When Danny is telling Paige and Cappalino about his "three" crimes, he describes a run-in he had with a local mobster. When he gives their names, the names he says are obviously dubbed in.

Anachronisms

Early in Ciello's work wearing a wire, Ciello and his wife go the the movies. The feature they are seeing is Ship of Fools (1965), but in the lobby, there are posters for All the President's Men (1976). However, later in the film, there is a montage revealing the recordings that date back to 1972, four years before the release of All the President's Men (1976).
Shortly before Danny goes to work for Cappalino, he and his partners are in a barber shop, and one can hear Captain and Tennile's version of "Love Will Keep Us Together" on the radio. Later shots of Danny's tapes show that he started working as an undercover cop in 1972. Captain and Tennile's version of "Love Will Keep Us Together" was released in 1975.

Character error

During the Blomberg appeal, the judge calls Detective Ciello "Lieutenant Ciello".
