After the famous "exploding head" scene, there wasn't a drop of blood on Revok or the table.
(at around 9 min) Man lying on a bed does not have shoes on in this shot. In all other shots he has white sneakers on.

Revealing mistakes

At the very beginning of the final fight between Revok and Cameron, Cameron hits Revok in the head with a statue. When the camera cuts to Revok's face before the statue makes contact, there is already blood on Revok's face where the statue is about to hit.
Visible wires pulling an actor through a window during the computer lab explosion.
Clearly visible stunt actors for both Cameron and Kim as the characters are briefly shown running away from the exploding phone booth.
When Cameron "scans" the mainframe computer from the gas station telephone booth, the handset and its cord are clearly wrapped in layers of black plastic gaffer's tape. This is present throughout the entire scene and was likely done in an attempt to camouflage the special effects used in the violent destruction of the handset at the end of the sequence.

Audio/visual unsynchronised

When the security escort guard is ordering his driver to slow down, he says "I'm going to kill you if you don't do it!" He is seen to mouth "stop" instead of "do it".
Dieter Tautz cries out during Cameron's scanning of him, but his lips don't move.

Crew or equipment visible

When Cameron Vale first visits Kim Obrist, before going up the stairs to meet her, some lighting equipment and the boom microphone is reflected in the glass door as it closes behind him.
Before the scene where Cameron is officially introduced to Kim at the apartment where all the good Scanners meet up, there is a zoom-in on Darryl who is waiting outside. You can see Michael Ironside is standing on what is the end of the dolly tracks which is used to smoothly truck the camera in for the zoom camera move on his face.

Character error

Kim asks Dr. Keller his name while he is interrogating her, and he refuses to answer. He clearly is wearing a name tag that says Keller on it. It appears to say "GBKeller".
