When the limo comes veering around the corner and swings back into the curb, the driver's side front wheel gets torn off. In the next scene, with the limo launching into the bay, the wheel appears intact with the car.
When Booker is jogging and his friend in the brown car pulls up, a Toyota Celica is shown passing the brown car. When the camera angle changes to the driver of the brown car, the Celica never appears outside his window as it should have. In the next scene, the same Toyota Celica is shown passing his friend's car as he pulls over.
When Chuck is driving the limo, aerial shots and shots of the passenger including the outside show they are on an isolated freeway. Shots of Chuck including the outside show they are in a residential neighborhood.
After Mike is shot on the ski slope, Booker rides the snowmobile down to chase the shooter. In the shot showing his view, everything is clear ahead of him. In the next shot, showing him from a bystander's viewpoint, Mike's yellow clad body is in full view.

Factual errors

When the Major Mihn (Soon-Tek Oh), the assassin at the airport, attempts to shoot Major Booker (Chuck Norris) he uses a revolver with a silencer. This is a classic 1970's action trope. Silencers can not work with revolvers since the noise escapes from not only the barrel but the gaps in the front and rear of the cylinder.
When the Black Tiger commandos get in the helicopter at the beginning of the mission, Major Booker (Chuck Norris) enters the front of the helicopter and sits in the right hand seat. US military helicopter pilot in command fly from the right hand seat. If Major Booker had sat in the front of the helicopter he would have sat in the left hand seat however, sitting in the front seat would not allow an easy tactical egress at the Landing Zone so he probably would have sat in the rear with his squad.
The scene where Norris and love interest lady are arriving at the top of the ski hill, a man can be seen wearing a Hawaiian t-shirt.

Revealing mistakes

In the newspapers that Saunders scans through, the articles below the shocking headlines is repeating text.
At 29:55 when Booker drives away from the tow truck, the tape markers designating where his car has to be are clearly visible on the road.

Crew or equipment visible

When Norris fights three men surrounding a car he wants to get into, a shot includes the cameraman in the car window reflection.

Errors in geography

Norris drives down a ski resort hill which leads to forest. The forest then turns into a desert and then later on the environment turns into a rainforest.

Plot holes

On two separate occasions Major Booker (Chuck Norris) is standing in very close proximity when his team members are assassinated by Mrs. Mihn (Virginia Wing). Both times Mrs. Mihn, supposedly an expert shot since at least one execution is done while the target is moving, allows Major Booker to escape without attempting a second shot.
