Fred's sawed-off high-caliber gun should leave bigger holes and cause more bleeding than what is shown - especially during the final shootout.
When Boss is being patched up by the good doctor, the doctor uses what appears to be medical tape to get the job done. However, medical tape was invented in the 1920's which was after the end of the Old West era.
Amos was smart enough not to have said where they were taking Boss while one of the bad guys could hear it.
