When Beth and Columbo are having coffee and cookies, the cookie she is eating keeps changing from the only light colored cookie on the plate to one of the brown cookies.
Beth is sitting on her bed, waiting for Bryce to come home. She opens a box of chocolates and reaches in for a piece. There are no wrappers on the bed at this point, but as she puts the piece of candy in her mouth, the bed is littered with candy wrappers.
During the questioning after Bryce Chadwick's death, the 2nd detective and Peter are sitting on one couch, and the 1st detective is sitting next to Beth on another couch, across from them. Peter tells the 2nd detective that he is in a relationship with Beth, and then there is a shot of her sitting by herself. Shortly after that, the 1st detective is seen sitting right next to her again.
When Beth leaves the hairdresser, her hairstyle is a flip. When she arrives home with Columbo, the hairstyle is more of a pageboy.
When Columbo is confronting Beth in her bedroom, there are several close-ups of her holding the gun, and it's under her left arm. You can see her left hand in the shots too. But when she pulls the gun on him in the long shot, she pulls it from her right side.

Factual errors

Marcia Wallace is not credited, even though she had a speaking part during the inquest. There may have been a mix-up during production, as she is credited in Murder by the Book (1971), the first Columbo episode, even though she doesn't appear in it.
When Columbo questions the suspect at her hair salon, the floor has a luxurious deep-pile carpet that may have helped set designers create a high-end feel. However, carpet and hair styling do not go together for reasons of hygiene. No salon, not even one in Beverly Hills, could get the necessary inspection and license to operate with carpeting in the same areas where they cut and style hair.

Incorrectly regarded as goofs

Columbo has to stand on a step ladder to retrieve the flower pot where the hidden key had been. It is unlikely anyone would hide a key in such a "difficult-to-retrieve" place. Although, it is more likely the step ladder was a plot device meant to highlight Columbo's somewhat diminutive stature. The hidden key could be reached by an average sized adult.

Revealing mistakes

Although Bryce is shot three times in the chest, no blood is seen on the body or the carpet after he is dragged across the room.
Beth throws the yellow bulb, and there is a loud shattering noise (with poorly synced sound). However, she threw it at a curtain, and the curtain didn't move.
When Columbo goes to Eugene (the spa), the receptionist tells him that his cigar's fragrance is not compatible. Columbo understands the inference, hands over his cigar to her, and says, "That is a shame; I just lit it." Yet there is no smoke coming from this "just lit" cigar.
When Columbo is in the bar, Peter comes over and talks with him. As Columbo talks, he is holding his cigar. When he takes a drag on the cigar, a sucking noise is heard, yet there is no burning ember on the end of the cigar nor is there any smoke.
When Beth imagines the crime she's about to commit and the police questioning her afterwards, the voice she hears is that of Garry Walberg, the actor who, in fact, plays the officer that questions her after the actual murder.

Miscellaneous

When Columbo notices the potted plants outside the door, he mentions finding the imprint of a key in one of the plants; he goes on to state it's like a fossil, being rained on. However the plant is in an alcove and would not have been exposed to rain.

Audio/visual unsynchronised

In the board room, when Beth removes her coat, she has her back to the camera and says, "Oh, thank you," to Columbo for nor apparent reason. Then, when she turns around, she repeats, "Oh, thank you," but her mouth doesn't move. The line of audio has simply been repeated.

Plot holes

She shoots Bryce, but there's no blood on the carpet. She then drags him to the French doors - the cops would surely notice the heel marks.

Boom mic visible

(at around 44 mins) When talking about the new blue car, a boom mic is reflected in the car's windscreen.

Character error

Peter Hamilton (Leslie Nielsen) says, "She didn't kill anyone. It was an accident." He should have said "She didn't murder anyone. It was an accident." She did "kill" her brother.
When Beth Chadwick starts to leave the hairdresser, she asks for her bill and for another appointment time. She is given her bill, which she signs, but she isn't given an appointment time.
When offered cream for his coffee, Columbo turns it down, saying he has his coffee black, but he already is seen drinking a white coffee.
