(at around 27 mins) In the first story, when the parts of the dismembered corpse are moving around, two right hands appear, one crawling on the floor and the second reaching down for Bonnie's face.
(at around 31 mins) When the landlord of the tailor holds up his rent book, it alternates hands between shots.
(at around 50 mins) After listening to his story, Dr. Martin is leaving the room of patient, Bruno the Tailor, and it can be clearly seen that the door is for room #33. After the door is shut and the camera angle shifts to the hallway, where we see Dr. Martin and the orderly, the door now clearly shows that it's room #32.
(at around 46 mins) In the 'Weird Tailor' episode Bruno and Smith are struggling for a gun. It goes off and Smith falls down dead with his face looking up but in a close up his face is to one side.
(at around 48 mins) When Anna says to Bruno that Mr. Smith knows about the suit after he tells her to burn the suit, she's not in the same position as in the previous shot.

Factual errors

There are two scenes where men are choked to death in the movie and in each the victim is choked too fast. Choking someone takes time.

Crew or equipment visible

(at around 27 mins) When Bonnie is being chased by a severed arm crawling along the floor, a crew member's hand is visible, holding the arm to make it appear as if it is moving.

Plot holes

All of the patients were under the care of Dr. Starr, but none of them recognize him in his new identity.

Boom mic visible

(at around 1h 1 min) When Lucy leaves Barbara's bedroom the first time, the shadow of a moving boom mic is visible against the white bedroom door.
