At the beginning of the movie in Shaft's office, Shaft changes back and forth between a lighter brown coat and a dark coat. The difference can be seen by noticing the horizontal seam on the back of each coat, one is straight and the other is not.
During Bumpy's visit to Shaft's office, the hat Bumpy is wearing changes. As Bumpy faces Shaft, the brim is turned down all the way around the hat. There is also a small gold buckle seen on the leather hat band. When the camera moves to Bumpy's left, to include Shaft, the hat's brim is turned up in back, and the buckle is missing.
When Shaft gets hit with the machine gun, one of the wounds is to his head - you can clearly see blood there. Yet after the doctor works on him and gets him patched up, all signs of the head wound have vanished - there's no scar or patch there.
In the beginning when Shaft is walking across a street, an overhead shot shows him walking past a taxi and nearly getting hit by a taxi. However, a street-level shot shows Shaft standing in front of and giving the finger to the brown car.
During the bar scene, the light in Shaft's apartment across the street shows a single window on the second floor. But the interior scene with Vic shows four windows on the ground floor.

Factual errors

The month calendar in Shaft's office is January 1971 (February 1971 is also posted on the wall in a "next month" style arrangement). But in Lt. Androzzi's office, the date calendar reads Tuesday, January 27 and later Thursday, January 29. January 27, 1971 was a Wednesday and January 29, 1971 was a Friday. January 27 and 29, 1970 fell on a Tuesday and a Thursday.

Revealing mistakes

Audio/visual unsynchronised

When Shaft pushes one of Bumpy's goons into his office, the sound of glass breaking is heard, but the glass window in the office door is clearly undamaged until a short time later.

Crew or equipment visible

The outline of a microphone is visible under Shaft's turtleneck at one point when Bumpy Jonas visits Shaft at his office.
