When the bus stops in front of one of the henchmen chasing Frank , the front door opens, but when they cut to the inside of the bus, the door is still closed, and remains closed, until Frank gets up and goes out the rear door.
After the cop guns down Chester in the drug store, a clerk arises from behind the counter to watch the proceedings, but she is missing in the cut-in shot of Frank rounding the front of the counter.
In the bar, the glass changes positions on the bar relative to the man with the checkered coat. It goes from behind his arm to in front of his elbow.
When Frank starts to sit down next to Jeanie at the club, his cigarette is in his left hand. But in the next shot, while he's still in the motion of sitting down, it's now in his right hand.
At 28:31 the way the doctor holds the paper changes.

Factual errors

Frank rushes into the emergency department of the hospital, which is a small room on an upper floor. An emergency department would obviously be a large room at street level.

Incorrectly regarded as goofs

When Frank leaves the St. Francis and gets on cable car 519, it is going Southbound towards the end of the line on Market Street, a few blocks away. In the next scene, cable car 521 is going uphill (Northbound) in the opposite direction, and he gets off five blocks away at California Street. Switching between cars to get to his destination is not shown in the film.

Revealing mistakes

After finding out who's in the photo, Bigelow leaves the photography studio and immediately starts getting shot at. He heads toward the factory (screen right) where the shots are supposed to be coming from, but all the shots being fired and ricocheting off the ground, pipe, barrel, etc. are coming from the other direction (screen left).
When Bigelow is being shot at in the factory, he returns fire once, with a single shot. At the end of the film, he shoots 6 times when he confronts Halliday. That's a total of 7 bullets fired, although the gun would have held 6.

Miscellaneous

On the door of Eugene Phillip's Import-Export business his last name is spelled with two "L's", but in the end credits his wife and brother's last names were spelled Philips with one "L."
During the chase in the drugstore, you can see several bystanders watching the filming from across the street.

Anachronisms

Frank rents a car for $2.00 per day plus mileage dated July 18, 1949. In a much later scene Frank is seen standing near a picture of cover of a September 12, 1949 a life magazine picture cover on a news stand.

Crew or equipment visible

Near the end of the first scene in Halliday's office, a large shadow or change in lighting briefly passes across Bigelow.
Near the end of the film when Frank revisits Mrs. Philips, we see several film lights in front of the building across the street as he parks his car. We see them again when he's in the parking garage as Majak and his henchman approach the building. The whole block was probably lit up for the chase scene that follows.
When Frank escapes from Chester's car, he runs down the street past a truck of unused lighting equipment.

Plot holes

When Dr. MacDonald is showing the beaker which is supposed to contain the luminous poison, which he has just removed from Frank's blood, there is more toxin in the beaker than could possibly have been in a small blood sample; more than Frank could have drunk in the 2 small sips he takes in the bar; more than could have been dissolved in the glass. Additionally, when Dr. MacDonald is showing the test tube containing the poison liquid that glows in the dark and moves it around, you can see it is really a solid substance, and not a liquid.
Bigelow goes to the doctor complaining of a stomach ache. A toxicologist ran tests showing he was poisoned by luminous toxin. A doctor would not run toxicological tests upon an initial patient visit with a stomach ache complaint.

Boom mic visible

While Frank and Halliday discuss Raymond Rakubian's photo, the mic shadow appears on the side office door, again above Frank's head.
Just before Paula slams the door after her argument with Frank, the boom mic shadow appears above Frank's head.

Character error

When Frank is being rushed out of the room after being captured by Chester, the short henchman briefly looks into the camera.
