When the gun is fired, it is pointed towards the floor, too low for a bullet to strike Mary in the back.
During the conversation between Dr. Carrol and the man from the FBI, there is mention of a case involving a young woman found in the company of five men. The FBI man introduces the case as being from Michigan, then later says that it took place in West Virginia.
During the high-speed drive immediately preceding the pedestrian accident, the car passes the same tire shop three times and the same billboard and gas station twice.
When Jimmy falls into the Lanes' fountain, his trousers are soaked. However, when he walks into his house a few minutes later, his trousers are completely dry.
When Mrs. Lane serves hot chocolate to Mary and Bill, the kettle spout is pointing in different directions between shots.

Revealing mistakes

The car that is "stopped" at the school crossing in the opening street scene is clearly not running at all, let alone just stopped by the policeman.
The same art deco ashtray that Mae Coleman has on top her piano (as well as the piano) also appears in the famous "Play Faster!" scene (where Ralph is in hiding from the cops and the girl plays piano for him while he's high).
In the very first street scene, extras in the background are waiting for their cue to walk along a sidewalk in the background.
When Dr. Caroll and the unnamed man behind the desk talk about marijuana, there is a door by the filing cabinet that has had a name removed from it (not done very well). In a later scene with Dr. Caroll and Bill, the same door appears next to bill. (Both scenes filmed on same set.)
The same extras used in the opening PTA (or TPA) scene are used in the courtroom scenes.

Audio/visual unsynchronised

When Jimmy falls into the Lanes' fountain and Mrs. Lane helps him out of it, he says "It's all right, I'm fine," etc. without moving his mouth.
