Lori is taking care of Ernie Dell, who wants his marriage to be annulled, because he thinks his wife cheats on him. Sally and Brad work on a case concerning a divorced couple and their kids.
- Meredith is horrified when her mother is admitted, and everyone is concerned when Cristina collapses in the operating room. Meanwhile, Addison is determined to save a very sick baby.

- When Ms. DiPesto has won a competition, she is allowed to join a Murder on the Orient Express (1974) inspired crime game in a train created by her favorite crime author. She is accompanied by Maddie and David to central station. Maddie and David miss to leave the train before it starts off and now have to stay on it since it won't stop for the next 24 hours. When a real murder happens, everybody is suspicious and the two detectives have to solve the crime.

The two detectives find this man, but find out on the next day that someone murdered him. Of course, the mysterious woman is their very first suspect.
