(at around 1h 55 mins) Near the end of the movie, the plane opens its rear door to drop the bomb. In one of the next shots the plane is shown with its rear door closed.
(at around 1h 16 mins) While Casey is having the seizure in the hospital, Sam rips his shirt to allow the nurse more space to place ice on his body. In the next shot his shirt is pushed up, not ripped.
(at around 1h 45 mins) When McClintock's helicopter goes under the bridge and splashes into the water while chasing Daniels' helicopter, the pilot can be seen wearing sunglasses. He is not wearing sunglasses in closeup shots.
(at around 20 mins) While arguing next to the cab, Keough has a statue under her arm, in the next shot it's gone.
At 01:48 when they run out to the chopper after validating the serum the missiles that was fired during the chase are still there.

Factual errors

There are no (naturally occurring) Capuchin monkeys in Africa.
(at around 1h 29 mins) Salt comments that their Loach (Hughes H-6) will go 400 miles without refueling. The Hughes H-6's real range is only 267 miles.
(at around 4 mins) In the beginning of the movie, as the camera passes through the various bio-threat labs, many doors are open to hallways and personnel not wearing masks, and one technician even removes her mask before leaving the lab.
Blue suits used in level 4 laboratories have a special air supply. The sound this makes inside the suit is so loud that you cannot hear other people. The people at the starting scene of the movie communicates none the less during the first scenes in the level 4 laboratory. Yellow suits are pressurized (positive pressure, which means air can go out if torn, but never in) via a small motor inside it and a set of batteries, but the yellow suits show no sign of being inflated at all, and when helmets are moved no air rushes out. It means also that Casey tearing his suit shouldn't have ended with infection.
A centrifuge lid has a safety locking mechanism that prevents it from being opened while the centrifuge is in motion. The centrifuge must first stop completely before the lock can disengage.

Incorrectly regarded as goofs

Hospital HVAC Systems are supposed to be designed to not recirculate air from room to room. The ideal design is so that patients who walk in with broken arms do not walk out with other maladies. However, many hospital HVAC systems are not correctly maintained, and others are not correctly designed - some/many have exam rooms that share air. So when the virus in the movie mutates and is air born, it could be spread by the HVAC System especially in a community hospital that isn't geared up for contagious diseases. In fact, the Lassa fever virus was spread exactly this way in a major US east coast medical center. One person, a tech working on a non-patient floor, contracted the virus even though extreme containment procedures were thought to be in place - the HVAC system was assumed to be the pathway.
(at around 26 mins) When Daniels, Schuler and Salt are first viewing the Motaba virus, the close up shown appears to be a picture of an Ebola Virus. Actually, it is not possible to discern the Ebola virus by its looks from other related viruses. Ebola belongs along with Marburg (and possibly a third, recently discovered virus) to the family of "filoviridae" (meaning "thread-like viruses") that all look the same. It's possible that the (fictional) Motaba virus belongs to the same family and hence looks the same.
The Fairchild C-123 Provider aircraft shown dropping the 'bomb' in the film was withdrawn as obsolete from the U.S. Air Force inventory at least 10 years before the period in which "Outbreak" is set. The scene in question however is set in 1967, about 13 years before the decommission of the C-123s. Additionally, it is not normally possible to hold a conversation aboard an in-flight C-123 due to the roar of the engines. We see Salt and Daniels converse while en route back to the US from Africa.
(at around 1h 22 mins) When Salt looks out the window and notices the troops leaving town they are next to the MPH sign and marching down the street. When he looks out again, after about two minutes, the troops are still marching, but their position has not changed.
Hoffman was able to get back on the helicopter from the ship even though it originally could not land. Salt told Hoffman to have the crew clear an area to land after he was on the ship.

Revealing mistakes

During the helicopter chase, one shot from inside the helicopter shows a blue window - the blue screen was not removed in post-production.
When Dustin Hoffman is preparing to board the aircraft to Zaire, and when he's at the airport preparing to go to Cedar Creek, the large cargo aircraft in both shots is in the same exact location, and the propellers, hangar doors and even some of the set dressings are in the same location, indicating those two scenes were shot on the same day.

Miscellaneous

The attribution for the epigram at the beginning of the movie punctuates Ph.D. without the second period (there should be both or neither, not just one).

Anachronisms

(at around 1 min) At the opening of the film, the setting is said to be "Zaire, 1967". The country was only called Zaire between 1971 and 1997. In 1967, the country was called the Democratic Republic of the Congo, which was also the name it reverted to after 1997.
The Commander of the U.S. Army Medical Research Institute of Infectious Diseases (USAMRIID) in 1995 when the present day of events takes place was a Colonel. Therefore, General Ford should rank at Colonel and Dr. Sam Daniels should hold a lower rank than Colonel.

Crew or equipment visible

During the helicopter chase, the shadow of the filming helicopter falls briefly on the rock face.
When Dr. Daniels is inspecting the room with the infected car accident victim, the cameraman is reflected in his helmet.
(at around 59 mins) The civilians killed attempting to escape quarantine in the pick-up truck from the military Hummers, are clearly dummies on fire.

Errors in geography

(at around 1h 26 mins) When Daniels and Salt talk to George at the San Francisco Federal Building, George says he is from Sioux City, SD. There is no Sioux City, SD. Sioux City is located in Iowa with the suburb of North Sioux City residing in South Dakota (not to be confused with the larger Sioux Falls, SD).
The H-6 only has a range of 267 miles. Looking at the direction as Daniels and Salt are flying the helicopter along the coast line, it is clear that they are flying south from Cedar Creek to San Francisco. Now, there are five Cedar Creeks north of San Francisco, the closest one located 180 miles northeast of San Francisco. This Cedar Creek is far from the coast though. The Cedar Creek that is closest to the coast is a whopping 323 miles away, so the flight to SFO and back plus standing in the air against the plane would never have worked without refueling. All of the Cedar Creeks are real creeks anyway, and uninhabited. When preparing for the way back to Cedar Creek Daniel mentions a town called Palisades between San Francisco and Cedar Creek. There is no such town.

Plot holes

The monkey in the pet store is infected at the same time that Rudy Alvarez (pet store worker) is infected and several hours before Alice (Jimbo's girlfriend) is infected at Logan Airport. The monkey has no antibodies and so should have died around the same time that Rudy died and long before Alice died. But the monkey is still alive when found by the CDC crew.
Brigadier General Billy Ford relieves Major General Donnie McClintock of command towards the end of the movie, prevent the destruction of Cedar Creek. Ford as a Brigadier General is of a lower rank to a Major General and commonly would be illegal to perform such an act. McClintock himself should not obey that order. The only exception could be under § 1088. Relief of a Commanding Officer by a Subordinate US Navy regulations. Therefore, McClintock could stay in command and issue the order.

Character error

(at around 38 mins) When Keough is trying to interview Jimbo in the hospital ICU, he is clearly intubated. Intubated patients can not talk due to the fact that the intubation tube passes through their vocal cords.
When Major Salt removes his helmet in the village, the elder assures Daniels that it's okay because the virus is not passed through the air. Not only is Daniels unlikely to take the man's word for it and bypass the isolation protocol he originally ordered, but the village would have been too small a sample size for anyone--especially an untrained elder--to observe the pathogen and be sure of how it is transmitted.
When Robbie is assisting a local doctor to perform an autopsy she tells him "maximum sharps precautions". Shortly thereafter she tells the doctor to pass her the scalpel so he can assist her instead and he does so directly hand-to-hand. No trained doctor would pass a sharp instrument in this fashion as the standard (and safest) method would be to do so using a passing tray to prevent simultaneous handling of the instrument and avoid possible injury.
Around 20 minutes in Gen. Ford insists that containment of the village is not necessary because of how quickly the infected die, yet at that point no one knew what animal passed (and would continue to pass) the infection to humans, and thus a response on the ground was still necessary. Further, the village elder told Daniels that the village's patient zero had gone into the woods with an unknown white man, who could also have been exposed to the pathogen and be spreading it in a larger city.
Nearing the end of the movie when Dr Daniels is panicking to get back into Cedar Creek, he leaves his car window open and people lean into the car through said window. This is not a good move as it has been established that the disease is now airborne and so he shouldn't have put the windows down and if he had to, he should have had his mask on. Even if none of the people outside the town are infected, this would be an unnecessary risk to take.
