When Killian is at the entrance to the control room, the gate opens before Sven has finished entering the security code.
In the helicopter, Ben says, "I will not kill helpless people"; in the game show with the original "uncensored" footage of the massacre he says, "I will not kill helpless human beings."
During the opening dance number for The Running Man game show, one spectator is seen twice in two different places at the same time.
When Richards, Weiss, and Laughlin are sent through the rocket cage tunnel at the beginning of the game show, there is a camera angle that obviously shows the rockets to be going in tandem (one tunnel). This cannot be the case as when Richards hits the safety net at the bottom, he would almost immediately be rear-ended by the following rocket (possibly ending Richards' game before it had begun).
When Richards returns to the studio to confront Killian, the television audience is able to continue watching the show even though all the camera operators and stage crew have run away.

Factual errors

In the faked version of the Bakersfield massacre, there are shots from angles (such as outside the helicopter) where no camera could be. Even if this is taken as evidence that the public is being deceived with a manipulated video, the uncensored version has similar shots, including one from Ben's point-of-view when he is knocked out, which is not a view the camera could see from.
When Fireball uses his flamethrower, it shoots a clean flame with no liquid fuel/napalm left to stick and burn. This means it's likely propane which instantly evaporates when released from pressure, but when Ben pulls the hose out of the top, liquid spews out.
When Ben is in the helicopter cockpit, the display that gives the helicopter's latitude and longitude is changing at a rate of one degree per second, an impossible speed for a helicopter.
At the beginning of the film, Weiss is shown looking at a computer monitor while a guard enters a code to deactivate the security system. He's wearing a shirt with the word "trusty" printed across it. The word that's much more frequently used is "trustee", because it has a more legal connotation.
When Killian is visualizing potential contestants for his show, the monitor displays a prisoner whose profession is listed as "Interpretor". The correct spelling is "Interpreter". Shortly afterwards, another prisoner is listed as a "Collective Framer", instead of a "Collective Farmer".

Incorrectly regarded as goofs

When Richards kills Fireball, he does this near the corpses of last year's winners; however, at TV time the crowd doesn't know this, as they can't see the dog tags around their necks, and the TV crew could easily have muted the transmission later. When Mic's film at the end shows the crowd to which the bodies belong, they show surprise. This is because they realize Killian has been lying to them.
It has been stated that Mic's group couldn't have obtained the photos of the deceased Whitman, Price, and Haddad which they included in their "Killian is Lying" montage shown to the studio audience in the film's final act. The deaths of the three previous "winners" were closely guarded secrets by ICS, so it was assumed that there were no surveillance cameras in the area where their remains were discovered and where Fireball met his fate, nor did Ben Richards or Amber carry any photo or video equipment. However, this assumption may be incorrect, as the audience is seen loudly cheering at Fireball's on-screen death, implying that they were able to see the entire fight and that there must have been cameras inside the room. And if there was footage of the fight, the resistance could have easily obtained a few screenshots of the dead runners.
While the chainsaw chain is not moving all the time when Ben fights Buzzsaw, that's normal; the chain doesn't move, even though the engine is running, unless the clutch is being held.
Ben Richards uses Amber's pass (it works as a passport) at the airport and the guard doesn't question why he's using her card or demand his card. This seems bizarre to modern audiences after the increased airport security post 9-11, but in the 1980s when this was filmed, this was a completely plausible occurrence.
When Richards kills Fireball near the corpses of last year's winners, the corpses are destroyed, so no footage of intact corpses could be shown at the end of the broadcast. However, since the fight with Fireball was broadcast live, it is possible that the resistance used screenshots of the skeletons from the broadcast. The audience overlooked them because of Killian's lies.

Revealing mistakes

Early in the movie, a commercial for "The Running Man" is shown. All of the scenes in the ads are clips from the "live" show featuring Ben Richards which obviously could not have been shot yet, as the ad predates the show.
Just before Subzero slices the gong in two, the breakline is visible.
When Ben gets a needle between his fingers before the show, you see the needle move into the tube instead in his hand.
In the prison, Ben says to a guard, "Give you a lift?" and picks him up, but it's clearly a dummy that he lifts.
Damon Killian's stunt double can be seen when Ben flings him into the Running Man starting rocket.

Miscellaneous

Dan the Janitor has the wrong ID on his chest. The photo is of a different person.
When Ben is fighting Buzzsaw and after knocking him off the motorcycle, he approaches Buzzsaw laying face down on the ground. Ben slowly reaches down to either check if he's alive or to finish him, but the frame switches and Buzzsaw is now face up and attacks Ben. There isn't enough time for that to occur in real time.
At one point the helicopter undercarriage cannons are firing at something in level flight. However the riot was on the ground so they should have been facing downward. This is because this piece of footage was taken from another feature film, King Kong (1976). Also, although there are muzzle flashes, we do not hear the gunshots.
When the old woman in the audience picks Ben Richards to get the next kill, the takers in the street start taking bets for Richards but put his odds at 100-1. This would make no sense to pay out $100 for every $1 bet especially considering that he just handily killed 2 stalkers and disabled a third.

Audio/visual unsynchronised

Just after Dynamo kills Weiss near the up-link box and Amber is thrown to the floor, the next cut shows Dynamo advancing towards Amber. He is speaking or singing, but there is no audio for his voice.
When Killian turns and shouts, "It's time to start running," he takes the microphone away from his mouth halfway through the phrase yet his voice remains the same.

Crew or equipment visible

When in the ice rink, Weiss is pushed along by Sub-Zero towards the camera. Just as Weiss almost hits the camera, an arm reaches out from the left-hand side to catch him.

Errors in geography

During the aerial attack on Bakersfield, the helicopter's computer shows the Bakersfield coordinates as Lat 45.04 N, Long 119.06 W. These coordinates are actually in the area of Umatilla National Forest of northeastern Oregon, just west of the 395 near Dale. Bakersfield, California is actually Lat 35.37 N, Long 119.02 W. Someone might have typoed the 35 to 45, but even then, the coordinates are south of the Maricopa Highway on Snapp Road.

Plot holes

Fireball's death is captured on the show, but the moments earlier when Amber discovered that the three "pardoned" prisoners were dead apparently weren't filmed even though the show is broadcast live - very convenient for a show that tends to hide the truth.
ICS did not dispose/hide the bodies of the "last season's winners".
When Ben confronted Amber about who she was and Amber revealed that she took over the apartment a month ago from the brother since he was getting "re-educated", since Ben used the same pass-code and it worked with no issues, wouldn't a new tenant either get a new pass-code or if not change the current one to be safe.

Character error

Damon argues at one point that television had been telling people what to do and how to think "for 50 years." That doesn't make sense for when the film was made (1987) OR when it is set (2018-2019). Fifty years before 1987 is 1937, and 50 years before 2019 is 1969. In reality, television began to have cultural power in the late 1940s and early 1950s (at least in the United States).
"Subzero" is misspelled "Zubzero" on the bet takers' chalkboard throughout the whole movie.
When Captain Freedom states "...code of the gladiators", either he is not aware of the "runners" having radioactive tracers in their bodies to ensure being found, or he just refused to accept the reality.
