During the Mexican Place scene, Toxie throws away one of the mobs in the kitchen. When he lands, he's covered in tortilla chips. In the next shot, he's covered in taco shells.
When Toxie jumps up the top of Bozo and Slug's stolen car, he grabs onto the car's roof racks. In subsequent shots of the chase, the roof racks disappear and reappear.
When Toxie and Sarah are having sex, Sarah kicks off her red shoes. But in the next shot, the shoes are back on her feet.
Just before Melvin has sex with Sarah, Sarah's red dress straps fall down off of her shoulders, yet are back up in the next shot.
When Melvin is putting the ice cream, chocolate sauce, and whip cream on one of the robber's faces; the bowls, bottles and the ice cream on the robber's face all keep changing position.
During the love montage there is a scene where Toxie jokingly puts a pylon on his head and Sara laughs, Sara's character is blind.
When Melvin jumps out the window and into the container of toxic waste, an obvious stunt double is seen jumping out of the windows - Melvin's hair is red and the stunt double's hair is blonde.
When Melvin dives out the window and into a vat of nuclear waste, you can clearly see him land on a black mattress in the reflection of a store window.
When Toxie kills one of the criminals by deep frying his hands, one shot of the grill shows it boiling but the hands nor arms are present.
There's clearly blood on the milkshake machine, which disappears when Toxie switches it on. Probably considered a continuity error by the filmmakers, while they were shooting multiple takes over this "death" scene.
When Toxie is chasing Julie, Julie runs into a room, locks the door and puts a plank of wood to secure the door. As she does, she cries and whimpers, yet her mouth doesn't move.
When the car goes off the cliff towards the end, you can a red van with crew members off to the side of the road.

During the Bozo and Toxie car fight, one shot shows the shadow of the camera and camera mount on the street.
In the beginning when the narrator introduces us to Melvin, he introduces him as "Melvin Furd". In all the other Toxic Avenger sequels Melvin's last name is "Junko".
As Toxie kneels down to comfort Sarah following the dinner brawl, the boom mic is visible for a few seconds at the top of the shot.
