When Dinozzo and David are interviewing PO2 Borrows on the Majestic, she has an Enlisted Surface Warfare device on her Coveralls. In the next scene, when she is at NCIS, there is no Enlisted Surface Warfare insignia on her NSUs.

Factual errors

Previous poster is out of date. Uniform regulations changed 2-3 years ago and E-6 and below now wear a Khaki shirt with Navy Blue Trousers. Khaki is no longer just for CPO's and "O's"
Mallard identifies the weapon that killed the NCIS agent as a KA-BAR model 1277, relying on the serrations on the blade. The Model 1277, however, has a straight, non-serrated blade.
At one point Gibbs refers to what he calls the "infirmary" aboard a Naval vessel; however, in the real world of the US Navy, that place is known, both officially and traditionally, as the sickbay.
Gibbs addresses Philip Bosch, the Naval Academy student on the beach, as "Ensign", however his class has not yet had their graduation ceremony. A Naval cadet holds the rank of midshipman, and a fourth year cadet is a Midshipman First Class; a midshipman does not officially become an Ensign until after their graduation ceremony. Specifically they are not an Ensign until they are sworn in by taking the oath of office to be a commissioned officer in the United States Navy, the oath is given at the very end of the graduation ceremony; only at that point are they an Ensign and a commissioned officer, so prior to that point the student would be addressed as Midshipman Bosch.

Incorrectly regarded as goofs

Abby matches the bullet fragment to Gibb's rifle. Unless his rifle has been in storage all that time, not possible. In twenty years, it would have been re-issued to a stream of marine snipers, who would train with it or use it on deployment. The wear would change the rifling marks. In twenty years, it would probably have had a worn barrel replaced at least once, completely changing the rifling marks.

Correction: The M40A1 rifle Gibbs used when he was a Scout Sniper, and he used to kill Pedro Hernandez, has been in his possession since he left the Marine Corps, he killed Pedro with it after he was discharged from the Marines meaning he must have purchased his service rifle from the Corps after discharge, snipers are sometimes allowed to do so. He has also used it on duty several times since joining NCIS, explaining how Abby was able to match the bullet to it.
Petty Officer Burrows appears several times in a dress uniform. The blouse is khaki colored. Prior to 2009, the only enlisted people to wear khakis were Chief Petty Officers E-7 to E-9. Petty Officer Burrows would have been wearing a dress white blouse if summertime or black blouse if winter. However, the Navy enacted a uniform change that took effect in 2009; sailors E-6 and below now wear a khaki top with navy blue (almost black) pants. Petty Officer Burrows' uniform is therefore correct. Further, this is not a dress uniform, it is an everyday working uniform entirely appropriate for the setting.

Revealing mistakes

When Gibbs asks Ducky if he was able to determine what kind of knife was used to kill Agent Macy, Ducky opens her autopsy report and points to his notation of the murder weapon being a Ka-Bar 12-77 Army combat knife. However the report he points to is not for Agent Macy, her service record report is on the left page; but the right page where Ducky points to is the autopsy report for Marine Private First Class Nick Chandler, right above where Ducky wrote "K. Bar 12-77" it states the cause of death was electrocution following torture. PFC Chandler was a Marine murdered in the season six episode Legend (Part I) (2009), which was Agent Macy's first appearance and the episode that served as a backdoor pilot to the spin-off Navy CIS: L.A. (2009).

Miscellaneous

When Gibbs finds the body on the beach there is a concrete bridge in the background, which apparently is crossing the beach and exiting in the ocean.

Character error

The officer who was killed in this story was named Lara Macy. On a couple of occasions, when people are talking rapidly, they accidentally call her "Officer Lacy" (for example, Abby, when speaking to Gibbs in her lab about Mexico).
Jason Dean was said to be a highly decorated Army Ranger before being dishonorably discharged. However his uniform decoration ribbons are not those of a highly decorated soldier, they are about typical for a special forces officer with the rank of 1st Lieutenant (O-2), though are less than typical of a Ranger who saw as much combat as his record showed; none of his commendations appear to be combat related as there are no combat "V" devices. His highest commendation is the Meritorious Service Medal awarded for "Outstanding meritorious achievement or service to the United States", and in the order of precedence it is the 11th highest military award. His next highest commendation is the Army Commendation Medal awarded for "Heroism, meritorious achievement, or meritorious service" and is the 14th highest award. His third highest is the Joint Service Achievement Medal awarded for "Meritorious service or achievement in either combat or noncombat situations based on sustained performance or specific achievement of a superlative nature but which does not warrant a Commendation Medal or higher." It is the 15th highest military award and the lowest commendation medal, the rest of his decorations are all achievement ribbons and unit citation ribbons. He also has Combat Infantry, Parachutist & Air Assault badges, but those are all typical for any Army Ranger.
