Sex & Nudity

  • It's implied that the combine make humans infertile.
  • A horse statue is in the game, genitals are visible.
  • Alyx Vance has visible underwear at the back of her jeans.
  • Eli makes occasional sexual references about Alyx having a crush on Gordon.
  • Barney calls headcrabs "headhumpers".
  • Dr. Kleiner makes some suggestive remarks about headcrabs.
  • Stalkers are fully nude, however their reproductive parts are surgically removed. Vaginas (sealed off, but still visible) and buttocks can still be seen.

Violence & Gore

  • There is some gore and dismemberment towards alien creatures.
  • You can see burned corpses that have their flesh burned down to the bone.
  • The game's violence, blood, and gore are a lot more disturbing and realistic looking than the original game (Half-Life 1). For modern standards, it might not be as shocking.
  • It was recently discovered that the rotten corpse model in the game was taken from a real forensic image of a body with it's face burned off. The only edits the game made were changing the hairstyle, giving it clothing, and removing the one remaining eye.
  • Some enemies can be split in half with bladed traps or landing saw blades at them with the Gravity Gun.
  • Very bloody there are a-lot of scenes with gore or gibs in it, I would say this game has severe amount violence the gun action isn't all that gory, but pretty bloody and also when you shoot combine they scream horrifically it can be very disturbing i would say this is a severe.
  • Definitely not as bloody and gory as half life 1, but it does look alot more realistic.
  • One of the only reasons the game is rated M is because of the zombies.
  • The zombies are pretty gory with visible organs.
  • You can only kill if it is self defense or mercy killing.
  • Bodies are seen hanged.
  • Blood spatters a little bit but it's mostly different colors such as yellow and green. There are zombies with headcrabs on their heads that shamble and moan and scream when injured. There are modern as well as science-fiction weaponry with which to kill enemies, who come in various forms. In a particular level, zombies may be crushed or incinerated by various traps. Enemies fly about when killed (ragdoll physics) and can be blown up by exploding barrels or grenades. Alien creatures have green or yellow blood. When enemies such as a Combine Soldier, a Metro Policeman, or allies such as civilians, medics, rebels, and refugees are attacked, they all bleed red blood.

Profanity

  • 'Fuck' is used once, but other noises humorously drown it out. This is the only Valve-developed title to use the word uncensored, as the company is known to deliberately censor or avoid using it.
  • When you throw a grenade at a sniper sometimes they will yell "Shit!". Though this is brief and hard to understand through their masks.
  • Shit is used a couple of times, but very little. Other mild profanities, such as: ass, damn , hell, and crap are also used.

Alcohol, Drugs & Smoking

  • Barney mentions that he owes Gordon a beer.
  • It's implied Freemans HEV suit uses morphine and a form of painkiller to heal him. This is in a medical context.
  • Sometimes Gordon's HEV suit will say "Morphine Administered." When he gets hurt.
  • Glass bottles can be found throughout the game, especially in City 17. However, it's only implied to be alcohol, so it may just be soda or water.

Frightening & Intense Scenes

  • There is plenty violence towards defenceless characters.
  • The Ravenholm section alone is extremely intense and scary with violence, gore and blood throughout. The game can get pretty gruesome even frightening and has very disturbing themes.
  • At some points in the game you can find citizens that have committed suicide, although these scenes are very rare and appear mostly in secrets.
  • The game has a depressing storyline.
  • Ravenholm is disturbing and terrifying.
  • The headcrab zombies scream sounds are terrifying. But if you reverse the audio you get to hear a man screaming in agony and begging for help.
  • The stalkers are very disturbing.
  • Ravenholm is not as scary as other people might leave you to believe.
  • The aliens are somewhat frightening, especially at nightime, and so are the Headcrabs. The Poison Headcrabs, which are black, large and have four legs, could be frightening to someone with arachnophobia.
  • Citizens cower as Metro Policemen search their apartments and are often unfairly and brutally killed by the Metro Policemen. The game's first two hours of playtime contains a reasonable amount of this and is pretty depressing and disturbing.
  • Certain civilians have Headcrabs on their head, which torture them - their screams are muffled by the Headcrabs which gain full control of them turning them into gory zombies which may appear disturbing to several sensitive players.
  • The Ravenholm sections can be quite frightening and tense and also brings the game into a survival horror type setting. Disturbing noises and music can be heard, one of them a woman screaming in terror. Some parts require you to stand your ground against fast zombies, especially on rooftops. You must defend yourself whilst they clamber up the drain pipes towards you, their screaming is also disturbing. On saying that about the fast zombies, coming face to face with them a first time may be quite a shock, as they are not your usual slow zombies.

Alcohol, Drugs & Smoking

  • At the very start of the game one of the citizens states that the Combine drugs the water from the city so that the humans forget all the horrible things that the Combine does to them, aside from amnesia it has no side-effects on the citizens.

Frightening & Intense Scenes

  • The Zombies are not actually undead, but alive, and are in constant agony and screaming.
