- Some of the antagonists in the game named "Reaper Nurses", although monsterous in behaviour, look perfectly female. Some have theorized that these beings are the embodiment of the five woman the player character sexually abused before gameplay.
- During the level "Torment Therapy", in the lobby at the start of the level, multiple Reaper Nurses are posed suggestively in the lobby and hallway displays.
- Bierce shows a fair amount of cleavage throughout the game.

- From Chapters 1-3, all antagonists always either pounce, eat, or straight up attack the player using weapons or fists. No actual active violence is seen. As for blood, it is seen in a few places, including lots of secret areas and the Murder Monkey's mouths and teeth.
Profanity

- Some monsters and characters insult the player during cutscenes and gameplay, using terms such as "idiot".

- Monsters, humans, and other creatures actively pursue the player throughout every level and jumpscare the player should they catch up to them.

- During a flashback sequence, the protagonist is shown to be an abusive unfaithful father who finds the issues with his wife and daughter to be an inconvenience, wishing they were born without them. Though he is shown to not intentionally show the same intentions for his daughter, he cheats on his wife and traps her in a situation that ends up in her death, with the daughter accidentally getting caught up in the same said situation.
- The level "The Carnevil" has claustrophobic hallways and stages.
