When Locke and Maddock head to the bus crash it is raining heavily but the officers do not use their wind shield wipers. They arrive at the crime scene and it has stopped raining and the ground appears wet but several other items in the area are visibly dry.
In the 35th minute when the nurse and Locke go to see Locke's wife in labour, the other police officer (Ryan) mysteriously disappears from the scene at the nurses' station.
In the 11th minute when the bus is on its side, there is no LED route info visible atop the front of the bus. When Locke approaches the bus in the 12th minute, the LED is lit. When he retreats, the LED is off again.
It's been reported as Continuity error that "In the 35th minute a nurse refers to Locke's wife Jean, as Jeannie.". This is not an error as Locke himself refers to Jean as Jeannie as well at some points in the film, Jeannie is more like a nickname to his wife's name.
In the 58th minute Rya shoots Maddox in this face and he falls back on the floor. As he is lying there, blood is spreading downward on his shirt. Gravity would prevent this, as his shoulders are closer to the floor than his stomach is.
At 51:04, the professor explains about a super moon and a blood moon, and that it happens every 9 years. Blood and Super Moons are not the the same. Also they happen more often than every 9 years. A blood moon occurs when the moon dips low in the sky, through Earth's shadow, which casts a reddish glow across it. A full moon that occurs on the moon's closest approach to Earth is called a super moon.
At the 52 minute mark there is a sweeping shot of the 1997 Philadelphia skyline that includes the Comcast Center building which wasn't built until 2008.
In the 73rd minute, Locke's car has a Pennsylvania license plate of three alpha characters and four numeric, all yellow on a blue background, which is correct. However, it would not have read 'Pennsylvania' but rather 'Keystone State' at the top.
At 48:00 there is a view looking west toward Philadelphia and the sunset behind it. The view is to the left and looking west alongside Benjamin Franklin bridge. A full moon can be seen to the right and above the bridge (to the Northwest). This is impossible since a full moon can only be seen opposite the sunset. Additionally, in the northern hemisphere, the Moon would be both to the south and in a position near the setting sun as shown in the movie, and it would be a waxing quarter moon.
In the 9th minute in the rain at night, the police car has no rear license plate.
It was reported as Factual Error that "Unlike Maddox, who wears a typical police officer's jacket, Locke wears a long coat, more akin to a detective's coat or a trench coat, which would not be considered proper uniform for a police officer." - however, by that time in the movie Locke and Maddox have been promoted to detectives.
It was reported as miscellaneous that at "The 7th minute the lightly burned pancakes would not produce the amount of smoke which they appear to do, nor would they set off a smoke alarm. The amount of smoke changes between shots. And the fire alarm would be ceiling-mounted, not wall mounted." - Actually though true that most fire alarms are ceiling mounted, residential units do have wall mounted alarms.
The following was reported as Factual Error: "In the 23rd minute police begin rounding up many black women, despite having a description of 'a black woman in a blue hoodie with dried blood on her hands.' They would not round up persons who do not match the description, and they surely would not arrest and book them." - however, that's what they do. Then later in the movie, black people protest against the police - apparently, they were behaving racist and it wasn't the only time.
When Locke reached the surface, he begins swimming towards some lights on shore, which appears about half a mile away (if not more). As he begins swimming, you can clearly see a strong current in a river, which he has to cross. Additionally, when he comes to shore, strong waves (as if they are from an ocean) are coming towards shore; 90° different from the strong river current.
At 5:50 TV shows the logo for Channel 2 Eyewitness News. There is no such station in Philadelphia, but there is a Channel 3 Eyewitness News.
In the opening scenes, a red and white Toronto streetcar is seen as the masked gunman is running through the city.
At 54'30, a note is passed between two people with "Scratch" misspelled as "Scatch"
In the 20th minute Locke radios dispatch and identifies himself as 'car 39.' His unit number on the police car reads 05102.
The scene is set in 1988, but there is a police chase scene around 22:27 with Crown Victorias that weren't made until the 1990s.
The brother-in-law's eye wear never changed.
In the 11th minute there a fire truck stopped on a sidewalk, without reason or explanation.
In 1997, present-day vehicles are visible outside the flower shop with Lockhart and his daughter. The same thing happens at the cemetery.
At 32:12 when the coroner is placing Rya's dismembered left hand into the body bag the bandage she had on in the previous scene is missing.
At 36:35 the TV says 22 KRXJ Action News. Movie takes place in Philadelphia, but TV stations east of the Mississippi river all have calls signs starting with W. Call signs starting with K are reserved for stations west of the Mississippi.
At 36:40 the reporter states it is the 9th Year Anniversary of The Market Street Murders. The actual death of the girl took place at the Broad and Girard stop along the Broad St./Orange line, but the deaths of all the involved victims, collectively, came to be known as the Market Street Murders.
At 5:50, it states on the news broadcast the address 13 Broad St. There is no such geographical location in Philadelphia.
Tommy had to struggle a bit to get over the locked gate at the air field. Maddox, who follows him later, and who walks with a pronounced limp from his broken leg 9 years earlier, seems to have navigated the gate-climb with little to no trouble.
